anyone who can recommend a civil surgeon here at FL?

hi, just reaching out for the gurls from FL who have already filed aos, im having my td shot taken at tyndall air base, if u guys can recommend a civil surgeon who can transfer my med records and my td shot paper to form I-693A, it would be of great help

Dr. L E Arnold will do it for 10 Dollars. He is in Texas. All you have to do is mail him the documents and he will transfer them for you and mail them back with in like a week.

Send a letter,you need to mail em your vaccination record from your foreign country anyway.

Write a check that is fine.

I didn't require an I-693a because my shots were completed as part of my medical. The people who use this Doctor say they call the office, leave a message, and then the office calls you back with the instructions.

So where mine and my medical exam was only 7 months ago.

I have still however heard of people getting an RFE due to that form being missing (even if their shots where compleate and their medical was still valid)

Thats why I figured it would be better to be on the safe side.

Absolutely.... it's a personal choice on whether to send one in or not. (If your vaccinations were complete, of course.)
