AOS I'm confused about some questions?

O.K. these might be some very basic questions so bare with me you all.

1) "Current USCIS Status" in Part #1: what do you put in that section if I entered using a K3 visa?

2) Part 2: which check box do I tick if I entered using a K3 visa?

3) I-94#: does this number start with an A?

4) Do I need to submit medical records? The reason I'm confused is because it says "When required, submit medican examination report ...". This, to me, sounds like I submit the medical report when I'm asked to. In other words, I submit the AOS (I-485) then they determine whether I need to supply medical reports or not. Is that true?

Hi! I'm not sure how you should answer questions 1 & 2. On question 3, the 1-94# does begin with an A (It will be the same number they give you on your green card & work permit.). On question 4) You will need to submit a new medical exam report if your last medical exam (the one you took in your country before your visa interview) is more than 1 years old. Whether or not you need a new exam you should also include the vaccine supplment.. I-693A (available on www.cdc.gov, search on VJ as well, I'm sure some one the link or form on the post).

Question 1. Put K3 Visa Holder in the box in part 1 of the form

Question 2. Check box a, submit a copy of the I-130 petition approval

Question 3. The I-94 number is not the A number!

The I-94 is the small white piece of paper the immigration officer at the airport stapled in your passport. It has a unique number. Use that one. On the form they ask for the A number right above the field for the I-94 number - two total different things.

About the medical - you had one done in your home country I assume? If that was less than 1 year ago you are good to go. Just submit a copy of the records. As a K3 I believe you had to get all required immunizations overseas already, so no need to submit a separate vaccination supplement, that's for K1 visa holders.

The I-94 card that I have recieved is identical to the sample one provided here:

In the front, (displayed in the sample), there's a "Departure Number with a 9 digits then 2 digits on the top left corner. In the back, (NOT displayed in the sample), they have a hand-written number (like this: A89XXXXXX). These are the only two numbers on the I-94 card. Which one do I use in the question that asks about I-94#, part #1 of the I-485 form?

Also, it asks about US Social Security #. I don't have that yet. The whole purpose of me submitting the I-485 is, along with to get a green card, is to get a social security number. Do I leave it blank?

Yes, use the number on the front of the I-94, the departure number, that is the correct one for the I-94 field on the AOS application.

Leave the field for the SSN blank if you don't have one yet. I didn't have one either when I submitted my paperwork and got it later on with my EAD card. When I had my interview the officer simply filled it in by hand.

For the medical, If it is within 12 months old at submission, that will be fine but you may need to have your vaccinations transcribed by a CS onto the I-693a (supplement). All vaccinations age specific will need to be done. 18 - 64 MMR TD and varicela. Also depending on how ####### your local office is, some are now requiring the TB skin test as opposed to the chest xrays that were done in the overseas medical.

You can try to submit the vaccination supplement you received overseas and see if this will pass but dont be surprised if you get asked for one from a CS here.

Just take a note but be prepared in case you are asked for these.

The immigration officer at the port of entry took the whole envelope that contained the medical examination I've finished abroad. I did not make a copy of that. Is there a way to retrieve that back or am I pretty much out of luck?

They always take the envelope. The whole file is with USCIS now. You are not supposed to open the envelope so you couldn't have made a copy anyway. Usually the doctors give the applicant a copy/ a sheet of paper that states you had the medical check or at least a form that shows you had all your vaccinations done.

Any chance you can call the doctor who did your medical and have him fax it to you? That's what I did because they didn't give me anything either.
