I-130 random questions...

So my husband (U.S. Citizen) is filing the I-130 for me (Canadian citizen)....

#7 Other names used: I assume I put my maiden name correct? Like would I put .........????

A) Smith, John Peter OR... B) Smith

#17 Does he list himself? Or does he list me? Or both?

#10 Alien Registration Number: I'm assuming since this is the first time we're ever filing anything, we put None right? (I heard leave nothing blank!)

#14 I (canadian) lived in the U.S for 4 years while attending university there. I had a student visa, but now am back in Canada. Since I am no longer there I don't put anything there correct?

#14b What do I put here? Are they referring to us? Or are they referring to if my husband was married before? Neither of us were married before..

Forgot one more thing.........

#21 HELP!?!??! Yes we're husband & wife but we're obviously not living together because we're apart doing this visa process. What do we put there? We never lived together anywhere.

#22 How do I know which consular we'll apply at? I'm in Southwestern Ontario...I'm guessing Montreal?
