I supposed to get my passport yesterday however I got problem with my birthcertificate. (my first name) (Franchezca) - it was blurred. They told me to go to my local civil registrar to have a birth certificate transcription. Unfortunately, I found out that My name is mispelled in their record it was Franchezka. But eversince I'm in school , I'm using Franchezca. They issue me a PETITION FOR CORRECTION OF CLERICAL ERROR IN THE CERTIFICATE OF LIVE BIRTH which is also granted the same day. It states there that the petition is granted and that the name Franchezka in the birth certificate is now corrected to Franchezca. But the annotated or corrected birth certificate will be issue in 2nd week of february. They told me to show the PETITION FOR CORRECTION OF CLERICAL ERROR IN THE CERTIFICATE OF LIVE BIRTH to Department of Foreign Affairs together with my original birth certificate with error so they can issue me a passport.

QUESTION:

I called up US Embassy and told me that the petition package is already there. They ask my name, address and birthday for verification purposes. They found out that My name is mispelled in their record, my year of birth is also wrong and a few correction on my address. They told me to fax a letter of request for correction on my name, year of birth and address together with my birth certificate

. However, there is an error on my birth certificate. Can I fax the original birth certificate wth error together with the Granted PETITION FOR CORRECTION.......? since I still don't have the corrected Birth Certificate. Anyway, I can get the corrected birth certificate NSO authenticated 2nd week of February which is okay because I can have it before interview. But I chose not to wait for february just to have my corrected birth certificate and I also need to correct my records in US embassy before they can schedule me for interview.

THANK YOU Guys! I really don't know what to do...

Yes, I would fax both the old certificate and the petion form. Too much is better than something missing.
