You will need to complete the I-134 affidavit of support printable here. In addition to the things asked for on the form, you will need your last 3 years tax transcripts OR copies of your federal income tax return + W-2's for the last 3 years. You can request the tax transcripts from the IRS here for FREE. They take at least 2 weeks to get once you fax them the form. If you do not make over the 125% poverty guideline (which is around 17k right now) or have assets to cover the difference you will need to find a family member or friend who will financially sponsor her. However, if you make well over the poverty guideline, you do not need to provide any asset information.

It would also be good to send any evidence you have of your relationship together such as boarding passes, passport stamp copies, letters, pictures, emails, phone bills, etc.

Also, this is an example of the Packet 3 contents from VANCOUVER, Canada. It will not be exactly the same as yours, but they will be similar.

Thank for the info. I have my past 3 years tax return. I just started a new job I hope this won't be a problem, and I make over ther poverty line, so do I still need to put down my asset or bank figures $$$ in the form? I prefer not to but if I must than i will. The I-134 need to be notized correct? What about DS-156 form and DS-230? DO i need to have these fill out?

The DS-156 and 230 will be filled out by your fiance. If you make well over the poverty line then you won't need your asset or bank account info. You do need the letter from employer and tax returns though (copies of returns) as well as copies of your W-2's. The fact that you just started a new job shouldn't be a problem. And yes it needs to be notarized, don't sign it until you're at the notary.

Please tell me what forms are included on Package 3 (Instructions package) from HCM Consulate.

HCM Consulate has on 01/02/2008 sent out this package to my fiancee and now after 4 weeks, we still have not received it yet.

Can we just print out the requested forms on line and turn them in by person at the HCM Consulate?

Please advise!

Thanks a lot in advance!
