DS-3025 VACCINTATION DOCUMENTATION WORKSHEET

I was wondering if anyone could help. My wife and I had our AOS interview yesterday and was told that the information we had on vaccinations was incomplete. When I sent my AOS package, I just sent the DS-3025 form that we received in my wife's country (the one with the large brown envelope).

Most of the information is filled out (i.e Name, birthdate, passport number, alien number, exam date, physician name and signature). However in the body of the document, it has nothing filled in for all the different vaccines and the results boxes are not checked.

I've been trying to find an example of what needs to be filled out. Can anyone help?

Also, do I need to take this form and get it re-done by a local civil surgeon in my area?

7/27/07: Mailed AOS package to Chicago Lockbox via USPS

7/29/07: Package was delivered to Chicago Lockbox (Day 1)

8/30/07: NOA1 received (I-485, I-765, I-131)(Day 33)

9/4/07: Checks Cleared (Day 38)

10/5/07: Biometrics (Day 69)

10/18/07: AP & EAD Approved (Day 82)

1/9/08: AOS Interview (Day 165)

1/9/08: RFE, need to provide I-693A (Day 165)

2/7/08: I-485 Approved (Day 194)

I would probably go to a CS here, take the worksheet with you. Also take any records of the vaccinations being done. I dont know why the panel physician didnt fill it out correctly but the USCIS will need to know that they have been done.

You could have the titre test which will show what you have been vaccinated against.

If the DS3025 was not filled in correctly then it won't help the CS fill in anything (although I'd take it along anyway). What you will need is to take your vaccination notes (from the beneficiaries' country) to a CS and get a vaccination supplement filled in from that. If these notes are not available (and she has had the required vaccinations) then she will need a blood test undertaken to test for antibodies. If she hasn't had the vaccinations then you will have to get them done (either at the CS office or your local doctor's).

Basically, you have to take the DS -form from the medical examination in the foreign country and have a civil surgeon transfer the date from one to another. give me a sec to find I-693A

Well, they can take it, by all means, but it hasn't been filled in hence they are now needing a vaccination supplement! :D
