Please help, Is the I-693A differnt to the DS-3025

Hi All,

Please can someone help me. I have searched the forums and I am still stuck.

I am about to send in my AOS pack and need to supply vaccine history only, I have a signed copy from my medical in London with the vaccines they did, problem is, I can not see I-693A on it anywhere? At the top it says U.S Department of State and says for use with the DS-2053.

Anyone think this might be all I need to send in with the AOS?

I called all DR's in CO from the INS website and not one would transfer onto a I-693A, all DR's said I needed another medical regardless of any print out I have stating otherwise and they all cost $250!!

Please can someone tell me what the I-693A looks like?

I had my DS form put onto the I693 Form.

There is a panel physician in Texas called L E Arnold that does it for ten dollars.

You dont even have to go and see him.

My medical exam is still good until July,I still had it done due to the fact of people still getting RFE's
