OF-167

In my fiance's packet 3 it stated that anyone submitting an affidavit of support for him would need to sign the OF-167. I have never heard of this form before. Is there anywhere you can download this form online? Does it need to be notarized? He is interviewing at the Vancouver consulate in Canada.

The OF-167 that my fiance received was the public charge information outlining what could be submitted as evidence with the I-134.

I thought it was just a sheet of information and not something that had to be returned to the embassy.

Well I read up a little more on it and for Vancouver, the person(s) providing the I-134 for the beneficiary must also read and sign + notarize the attachment of the OF-167. This was some unexpected paperwork + mailing, but I don't think it's going to delay anything. (I hope! *fingers crossed*)

I also do not believe it can be found anywhere online, as it is different for every consulate...I've looked and looked and I can't find it so I'm going to have to wait for David (Canadian citizen) to mail it to me

Standard form that Canada consulates use. Maybe even Vancouver and not Montreal. I can't remember. I don't remember it having to be notorized though. Just an outline of what a public charge means etc. and your acknowledgement that you understand what this means. Wait until you get the from him.
