Application for Certified Copy of Marriage License

I am thinking of keeping my Maiden last name as middle name... and adopting Oliver's last name...

However, our marriage certificate has our names just the way they were... they didn't give me the option of changing that..

So now, when I go about filling out the form for requesting certified copies.. they ask me this:

FULL NAME OF BRIDE _____________________________________ (do I enter my full name plus my *new* last name here or not?)

BRIDE'S MAIDEN NAME _____________________________

I was and still am Lizelle F. L. L****

I wanna be Lizelle F. L. L**** K*******

or does the form have to match what the marriage certificate says.... and I have to do a name change later on?

In my opinion you should enter F. L. L**** K******* on the form and your maiden name where requested.

I do think, however, that your certified copy will probably arrive in your maiden name. My experience is that your marriage certificate will always appear in your maiden name, because, as you mentioned, it was registered that way - and that is normal.

As far as changing your name, you shouldn't have a problem, once you get your certificate, changing your other ID, etc with a combination of your marriage license and a request that you are now known as F. L. L**** K*******.

After working in a Registry, I can tell you that in Canada at least, all marriage certificates have the maiden name on them. That's because when you got married that was your last name. The change of name went on after that with the Driver's Licence and other ID. I also know that in Alberta, in order to take your maiden name as a middle name it had to be done as a legal change of name, and actually change the name on your birth certificate. That might be something you want to look into, just to make sure. It can be a big hassle. Some places will let you hyphenate your last names without the hyphen. I've seen many women do this and simply use their maiden name as a "middle" name. It also allows you to go by either last name or both. That's how it works here, I'm not sure how it is there, but it's something to look into!

January 5, 2009- I-485 transferred to CSC. Here's hoping for no interview!

January 14, 2009- Biometrics

February 23, 2009- EAD and AP received in the mail, dated Feb 14th.

April 23, 2009- Welcome to the United States Letter arrives. Card to follow.

June 1, 2009- GC received in mail. Approval date 04/09/09

Done with USCIS until 04/2011!
