Number 18 on the DS156 refers to the foreign applicant's spouse.

If you are the foreign applicant, your husband's full name goes there.

If your husband is the foreign applicant, your full name goes there including all surnames used, with emphasis on "used". For instance, my wife has been married but has never used either husband's surname.

If Jane Ann Doe married Mr. Jones, then Mr. Smith and now Mr. Wilson, and she were the wife of the foreign applicant, her name woudl be written as "Jane Ann Doe, Jones, Smith, Wilson". If the same person was the foreign applicant, and her name changed with the most recent marriage, the surnames Doe, Jones and Smith would be written in number 8. and her passport should reflect Wilson, so Wilson would appear in number 6.

Yesm all surnames you ever actually "used" including the one you are currently using.
