I have been look at the DS-2001 the check list, If she doesnt; have military records or she never been married. Does she still have to put a "check mark" in the box? Also my other question is that on the second page it says she will apply with: alone, with my spouse, and with minor children. Where is says apply with spouse, can she cross the word "spouse" out and put "fiance" there??

Help please. Thank You
