I applied as a K-1 and have already had my medical completed. Ive been busy all day trying to get the vaccination supplement signed by a USCIS approved doctor. After about 4 visits i finally managed to get it signed but she forgot to tick the section 3

3. Results

* Applicant may be eligible for blanket waiver as indicated above

* Applicant will request an individual waiver based on religious or moral convictions.

* Vaccine history complete for each vaccine, all requirements met.

* Applicant does not meet immunization requirements.

Which one did most people have selected on your forms? I have all the required vaccines, but i am also eligible for blank waiver's for some.... Also did you get the supplement form in a confidential envelope or was just a signed letter + stamp.

It should be in a sealed envelope and it should be ticked to say

I would go back and get it fixed.... asap

im actually confused myself because on my DS-0230 vaccinatino WORKSHEET i recieved during my medical in canada it says "Applicant may be eligible for blanket waiver as mentioned above" and that worked out fine... perhaps other K1ers can clarify whats written on their vaccination supplement from their local civil surgeons in usa

Your vaccination supplement should be signed (and stamped) by the doctor and placed in a sealed envelope. If all vaccination requirements are met, then the third box will be checked (all requirements met). If for some reason the applicant has not received all the vaccines because it is medically inappropriate (the reasons are listed on your form: not age appropriate, insufficient time interval (between two vaccines); not (flu) season) the applicant's form will have the first box checked (eligible under for blanket waiver). You should not have any problems if you have either one of the two checked.

Mine is checked on the third option (all requirements met), but still has some vaccines in blank with the observation "not age appropriate" ticked on the blank waiver's box...

Is that right???
