Wife lost her passport

Hello, my wife lost her passport that she had issued to her before we were married. To be honest I don't even know if it's valid anymore. Again, very hard to communicate 7000 miles away and her english is very poor.

So now that we are married, does she just apply for a new passport or does she have to apply for a lost passport with it being amended with a new last name? I have to get one for her daughter as well in which I believe never had one issued to her. The child is 10 yrs old.

I am kind of confused to where she goes for this. Does she have to go to the US Embassy in Manila or can she go somewhere closer to where she lives. Or does she go to the one of the Philippine Government Offices locally in which she lives? Which office would this be? My wife lives with her daughter in Bacolod City which is a long way from Manila. If someone could lead me in the right direction it would be greatly appreciated.

GUIDELINES ON PASSPORT APPLICATION
REQUIREMENTS FOR REPLACEMENT OF PASSPORT

Personal Appearance is required.

If the lost passport is still valid, please submit Police report

Submit same documents required for first time passport applicants.

*Please take note that there will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.

Ask your what DFA Regional Office did she went to when she first applied for her passport ( the one that got lost).

If it's in their Region VI, then this is the address.

For married women, who want to use surname of their spouse:

Personal Appearance is required

Marriage Contract (MC) in Security Paper issued by NSO or CTC issued by the Local Civil Registrar duly authenticated by NSO.

Thank you for the information. It sounds like she has to apply for a lost passport and have my last name amended to her new replacement. I think she went locally to get it or close. She can go to ILOILO I see that they have on there list of offices. This would be the closest.

There are better ways of getting your wife's passport changed/updated than "coincidentally" losing it after getting married (((wink wink))) ;) hehehehe!!!
