You could have gotten a photocopy of the DS-3025 at the medical exam, before the original was sealed-up for the consulate.

okay i am really confused. the medical for k1 went to the POE. all i have is the x-ray. i wasn't allowed to see the thing, so how can i have a copy to show the civil surgeon for I-693 so that i don't have to redo it all?

Indeed. Some civil surgeons will automatically give you a copy (like mine), or will give you a copy when asked. Methinks this is a good thing to suggest to those waiting to go to their medical -- make sure you ask for a copy while at the office! :)

good grief... ugh... she said i was not allowed to see it, so i didn't think about getting a copy. i did the exam and then the blood and xray results came in like 2 weeks later, which she picked up from the hospital herself. she compiled the stuff and fedexed it to the consulate as well. i thought that since i did it for K1 i would only have to make a notation of that for AOS. won't they have my K1 file in their possession when i interview for AOS? i dunno what i am supposed to do now???? i guess i can do a blood test that will show that i've had all my shots or something? ARGHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHH

Timeline

The physical that was done outside the US was done by a panel physician chosen by the Department of State. The Civil Surgeon is here in the US and is chosen my the Homeland Security folks. They do not play together. You should have been given the DS 3025 out of the package that you brought to the port of entry. You should contact the panel physician that you went to and see if you can get a copy of the DS 3025 vaccination record.

I've been racking my brains about this and I'm 90% sure we didn't pay anything for the privilege of having him fill out the form. The Civil Surgeon we saw just asked us to pay for the cost of the vaccinations we needed. (I think they were about $50)

That was it!

He was in Laguna Niguel, Ca if I remember correctly.

I get really jealous with you all, since I paid $ 250. That's only filling out I-693 form of AOS, blood test and X-Ray without any vaccinations anymore. It's because my vaccination is still new from my home country.

That medical clinic really rips me off. You think so? That's annoying!!!!!! And I was surprised the cost you guys paid for medical exam.... Gosh...

Red Cherry.. it sounds to me what you got was the full medical.. what they are discussing is just the vaccination supplement (I-693A) which is mostly just a list of vaccinations received.. and if they needed more they were charged for those as well... 250 for a medical is a little bit high but not too bad at all...

I already paid for USD 250 at the chasier, the doctor just recommended to have medical again. Obviously, the doc. doesn't know about the payment. So, I can say USD 250 is whether the doctor ONLY fill out the form or with/without medical exam. Thanks for your thought, anyway.

July 25 : Got a GC in the mailbox. Wow...what a real relief! My status is now clear:)
