$330. The shots I got did not hurt as much as this amount that I paid.

Wow! And I thought ours was high at $185. This was for the Vaccination Supplemental form and a chicken pox vaccine...Daylight robbery!

We paid 40 dollars for one shot vaccination with civil surgeon sealed from their office they gave me xerox copy.

We paid $40 for one a shot to complete my vaccination supplement sealed and they gave me a xerox copy.It's worth driving for 2 hours then paying 350 and others will charge us 450..urrrghhh!

Hi everyone,

I'm starting to get together everything I need for my hubby's AOS interview on the 23rd. We need a vaccination suplement and have been calling around for quotes. All his shots are up-to-date. The cheapest rate I have been able to find is $60. Can you all give me a sense of how much you paid? (Just the supplement not exam or vaccinations). Money is tight and every little penny we can save helps. I need to get a sense as to whether that is a reasonable rate just to fill out some paperwork.

We are in the San Francisco Bay Area FYI.

For the AOS/EAD filing I did not include a vaccination supplement but the vaccination worksheet. I did not get an RFE. THe instructions for the AOS interview say that a vaccination supplement should be brought to the interview. We don't need a complete medical because he had his exam during the K1 process less than a year ago. You can get immunizations done anywhere, just make sure you get them documented. The vaccination sheet, however, needs to be filled out by a civil surgeon. The list of civil surgeons can be found here.

$50. they just copied the worksheet (which was marked incomplete) and gave me the supplement (which was marked complete) in a sealed envelope as well as a photocopy. Took less then 10 minutes.

I get really jealous with you all, since I paid $ 250. That's only filling out I-693 form of AOS, blood test and X-Ray without any vaccinations anymore. It's because my vaccination is still new from my home country.

That medical clinic really rips me off. You think so? That's annoying!!!!!! And I was surprised the cost you guys paid for medical exam.... Gosh...

July 25 : Got a GC in the mailbox. Wow...what a real relief! My status is now clear:)

We paid $25 for the td shot, but another $50 for the 10 minute appt. fee, I guess that covers the doctor's time to review the old record to see what's needed, what's not, etc.

8/1/05: K-1 visa approved!

12/3/05: My honey and I entered through Chicago O'Hare together. She is finally home with me!

I-485 AOS

12/21/05: Got married

12/22/05: Applied for SSN, had to do manual verification, 6-8 wks:(; completed I-693 vaccination supplement w/ local civial surgeon

1/09/06: Got SSN card in mail, this is fast!

1/20/06: Sent AOS, EAD and AP package to Chicago Lockbox

1/31/06: Received NOA1's for AOS, EAD & AP

2/3/06: Biometric appt letter arrived today (wow! that's fast!), scheduled for Wed. 2/22/06

2/22/06: Biometric done, but found out we had the wrong DOB filled out on her I-485, cannot be corrected at the time because her case isn't in the "system" yet. This is stressful.

2/25/06: I-485 touched

3/6/06: Infopass Appt in downtown Chicago to resolve the DOB issue. Had to send a letter to the processing center.

3/24/06: AP approved and sent in mail!

3/27/06: EAD touched.

3/28/06: EAD approved online

3/29/06: AP received in mail

4/29/06: AOS NOA2 received -- interview date June 28 2006. Online status shows still in process.

6/28/06: AOS INTERVIEW in downtown Chicago Sucess! Passport got stamped and Green Card is on its way!

7/8/06: Green Card arrived

4/30/08: NOA1 date

You should have gotten the X ray and the DS 3025 vaccination document from the medical examination when you entered the country. At some point you will probably be required to go to an American doctor deemed by the government as a " civil surgeon ". He / She will convert the DS 3025 to an I 693A and certify that you have had all of the vaccinations you need. Some people are able to pass through the system with only the DS 3025, but others receive RFE for this document. There is obviously a large number of unscrupulous civil surgeons that charge great fee's to fill a simple form.

Red Cherry.. it sounds to me what you got was the full medical.. what they are discussing is just the vaccination supplement (I-693A) which is mostly just a list of vaccinations received.. and if they needed more they were charged for those as well... 250 for a medical is a little bit high but not too bad at all...
