My wife and i are about to fill out the forms for Adjustment of Status. She has chosen to take my last name but im not sure if we should use her old last name or her new one on the forms we are filling out.

Which should we use?

I-129F Sent 05-11-07

NOA1 05-31-07

NOA2 09-28-07

Left NVC 11-05-07

Packet 4 Received 11-16-07

Medical Exam 11-29-07

Interview Date 12-06-07

Visa Recieved 12-11-07

US Entry 12-19-07

Marriage 12-24-07

Oh she also wants to change her middle name to her old last name,which is a common thing to do in the philippines.

Should we use her full new name on all these forms?

Oh also can she apply for a SSN and state ID now before the packet is sent in?

Use her married name.

She can apply for an SSN, provided the 90 day 1-94 has not yet expired, otherwise she will need to wait until she has her EAD.

State ID depends on the state laws, if you apply for it now it may expire at the end of the I-94.

Im not 100% sure what i-94 is :unsure:=)

Where would i find the I-94 number for this i-485 form?

Should i wait till she has a SSN to file these forms or just leave it blank?
