Where to File AOS ?

Hi Vj PPL

First of all I live in Minneapolis Minnesota & I would like to know where do we file for AOS ?

I would also like to know what forms do I have to filled out ?

Do we have to send evidence also in the package when filing for AOS ?

I've read the guides but it seems confusing ,Plz or advise me on this one ........Thank You !!

Send application for 485, AP and EAD to Chicago

Only evidence needed to be sent to them is:

• Copy of foreign Birth Cert
• Copy of marriage cert
• Copy of I-94
• Copy of visa and bio page from foreign passport.
• Copies of US Citizen's financial evidence used for I-864
• Foreign vaccination record transfered to a supplement by US Civil Surgeon
