My I-693 came without the passport number too!!! And they have the info on the DS-3025!!!

Im calling her again also!!!

Aye, I wasn't sure what to think about that. She told me the same thing, but in the end, just to be safe, we decided we should phone back and give her the passport number.

We haven't gotten our corrected ones back yet—we only asked for them on Wednesday, though.

My wife's I-693A arrived this morning without the passport number.

I called the offfice and was told that they do not include it on the form.

Will this cause any problems with the USCIS?

I responded on the other thread, but thought I'd do the same here. I had my medical exam done by Dr. O'Keefe, and the passport number was not included in the paperwork. I was fine! He really is a nice man -- I'm disappointed to hear that he'll be closing shop.

After days of looking around my region here in Florida and hitting brick wall after brick wall, I decided to call Denise. I told her I knew this was short notice, because of their closing and asked if there was anything that could be done. She took my info over the phone, I faxed her the vaccination worksheet, and overnighted the $35 money order.

She is sending out the I693 A today. She was very pleasant as many of you have said. All in all, in cost me the fee, plus the $14 for the overnight. Much better than the aggravation I was getting here. Thanks for the heads up and truly sad to see them closing.

WOW! only $35...i had mine done with some chinese doctor and he charged me $65 for the supplement form since i had my medical march of this year. That sucks!!! Anyways...what's done is done. I just got my NOA1 for both AOS and EAD.

Just got my form in the mail yesterday, like the others, no passport number, but I'm sending it that way anyways!
