I-693 or Supplimental form to I-693

Hi...

Today my husband and I went to visit the Civil Surgeon here in our area. I had brought with me my x-ray film, vaccination records and the I-693 Form which we ordered from USCIS. When we go there, we totally got confused because there's 2 things the receptionist was saying. She said with the I-693 Form the doctor will do a thorough Medical Examination and I'll be doing the laboratory & diagnostic examinations, Then there's the Supplimental Form for the I-693 which is like that of the Vaccination Documentation Worksheet given by the authorized hospital of the US Immigration in our country.

Before getting an interview (I'm under K3 Visa) I had to go through that Medical Examination here in the Philippines and were given vaccines. I was told to just get a Td shot since it's not available in the Philippines other than that I'm good to go. Today, my husband and I are not sure if we did the right thing, because we picked the Supplimental Form for I-693 since I have gone through the medical examination last month. BUT now, as I read through the process of filing for an AOS & EAD that we need the I-693 (oh by the way, the medical result for the exam I had last month was in that brown envelop which the Immigration Officer took at the POE and was told that they'll be the one handling it). Did we mess up bigtime ?!?!?! The receptionist said that some people who go through them and who had their medical examination at the country where they came from only needed the Supplimental Form to I-693. She did seal the form and she also gave me a copy of the form. The only changes made was that they recorded my Td shot (which cost $105 by the way )

Which one is it?!?!?! So totally confused about this whole thing To everyone who will read and reply to my quiry....THANK YOU SOOOOO MUCH...

Your medical exam is good for 1 year from the date it was given. The supplemental to the I-693 (Vaccination sheet) fill out by a USCIS civial surgeon is all You need.

Are all adjustment of status applicants required to have a medical examination?

Most applicants for adjustment of status are required to have a medical examination. The medical examination must be conducted by a civil surgeon who has been designated by the USCIS. The examination must be documented using Form I-693, Medical Examination of Aliens Seeking Adjustment of Status.

EXCEPTIONS:

2. Ks: If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon.

my fiance got her k1 visa and arrived to States 2 weeks ago. the thai embassy has all of her medical examination records and other k1 stuff that i sent her prior to her k1 interview. what should i do to obtain those records? do i need to contact the UCIS here?

the US embassy will generally enclose the medical records in the sealed fiancee visa packet and the packet is picked up at the airport when the fiance arrives to the US.

For fiance visa applicants who are applying for AOS, you just need to send in the I-693A supplement.
