I'm filing for my husband and in section 4 information about myself I'm not sure whether or not to put maiden only, married name only or both married with maiden name in parenthesis (). Does anyone know for sure how they want it on there? We all know how picky they can be!! :wacko::help::help:

What name is currently on your social security card and/or US passport? If' you've moved since doing AOS, what name did you use on the I-865? That's the lastt name I would use.

where it asks for other names used that would be where you list your maiden name....

Not true in this case, since the OP is not the immigrant, but the USC. There is no place on the form for other names used by the USC. The other names used field is only for the immigrant.
