I derive an annual income of:

(If self-employed, I have attached a copy of my last incometax return or report of commercial rating concern which I certify to be true and correctto the best of my knowledge and belief. See instructions for nature of evidence of net worth to besubmitted"

-- Since i am working for a company and already have 3 latest payroll statements & a letter from the company stating my postion, salary, start date, etc on company letterhead.

Does this mean i do not have to send my W2's & last 3 year of tax statements (Federal Tax Return)??

I also have my latest checking & savings account statements. Is that good enough also instead of getting some letter from my bank?

AND also

Do i have to fill out the i-134 infront of the notary person? Or can i have it all filled out before i go and he/she can stamp it and date it??

I got a letter from my bankit was no problem. I am self empolyed I am bringing tax returns last three years .
