FAQs: Forms I-693 and DS-3025

Everything in my DS-3025 form is filled out correctly, as described in this forum (and there's a record of me having all the required vaccinations). But there's one issue: In Section 2, Results, there is nothing checked (there are five blank check boxes). Has anyone experienced this? Do I have to see a CS just to get a check in one of these boxes, or could my form pass? Any feedback or advice would be appreciated.

Your form is not filled out properly. There are no results. I would think strongly that you need a new form (I-693) filled out by a civil surgeon.

Finally found a CS who would transcribe the DS-3025 onto a Form I-693 without requiring an additional medical exam for my K1 wife and my K2 son!

I had called nearly all of the Columbus area Civil Surgeons, and each location told me that the doctor would require the physical exam (and exam fee) as a part of the process in getting the I-693.

Dr. Ronald J. Bloomfield did the vaccination transcription for $40 (x 2 =$80). When I called his office on the phone, his receptionist had also told me on that he would require the physical exam (at $200), but I then went down there in person just to try and learn if my wife and son's immunizations were caught-up. The receptionist looked at our DS-3025, then she took the forms and showed them to the doctor. Dr Bloomfield had full understanding of our needs, and 20 minutes later I was back in my car with two sealed envelopes that were stamped "Supplement I-693".

I had an appointment next Monday with Dr. Benson in Akron, OH. (who will also do the transcription only), but now I am happy to share this Columbus, OH Civil Surgeon with the VJ community:

Thank you for your response. I will follow your advice. I called around and found one doctor, who is willing to transcribe the vaccination into the I-693 form for 100 Dollars. All the other doctors wanted to talk me into the full medical examination. So I surrender to spending another 100 dollars and have my fingers crossed that everything will work out fine with my AOS. Thanks again.

My DS-3025 Result is "Vaccine History Incomplete," then "Applicant may be eligible for blanket waiver because vaccination not medically appropriate" check box is checked. So upon reading in this forum, I need to see a Civil Surgeon to transcribe it to I-693, my question is, do Civil Surgeon already have this form, or do I need to print it for them?? LOL. Silly question it is.

Hi. Apologies if this question has been asked already but I am currently in the process of filling in my forms for AOS and I need my vaccination record (I-693) updated to reflect my now complete vaccination history. I see from the FAQ sticky that it may be possible to have this done over the phone by a doctor in TX but I do not see the name listed on the USCIS Civil Surgeon Locator webpage. Does anybody know whether it is still possible to use this doctor, or should I just stick to those on the official list?

Advice appreciated. Thanks!

Hello Dear Visa Journey friends.

I am so happy becuase after 5 months I am with my fiance in the US. Soon I will apply for AOS.

Before that I have some question according to DS-2053. I am confused, should I attached this form or go to CS and send with my AOS new I-693.

Well, in my vaccination documentation worksheet I can find information that:

1. my last TD was in 03.27.2003 so is "insufficient time interval"

2. MMR 10/12/1999 is complited, Measles complited but Mumps is blank. Also my wonderful doctor mark on the begining that everything is contraindicated. Is it becuase Mumps is blank?

3. Finally Influenza is contraindicated because I did this form in September. So should I have flu shot when I will apply for AOS in October?

So my Vaccine HIstory is Incomplete with applicant maybe eligible for blanket waiver

Should I go to CS or my DS-2053 I good to send.

PS. I will be grateful if somebody could recommend me CS from San Antonio, Texas

Greetings,

My wife and I are currently working on all of our AOS forms. She recently arrived from the Philippines on a K1 Visa and received her medical exam over seas. From what I've been reading I can just submit our DS-3025 vaccination form without an I-693 as long as it's complete. To that effect our DS-3025 has everything marked appropriately except one column, rotavirus. This column is completely blank, is that going to be a problem?
