hey guys ....

i just found out about this thread and im glad i did in time :-) i havent managed to read the whole thread yet but i see im at the right place.

Anyway, we are in TX. ... does anyone know any good CS in TX?

I'm glad you think you found the right thread to read :) It's confusing as to what is "complete" and "incomplete" vaccination, whether DS-3025 alone is sufficient or do we need an I-693. First, thank you for the welcome :-)

Sorry about not having the timeline, but since both me and my husband have a username on VJ, we chose to go with only one timeline which is on his profile ( T&N ), so we wont confuse other members on Igor`s list. We just got married and now we are looking into the documents we need for AOS, EAD ... We dont know if we should simply go and file the AOS without the form I-693 or if we must get that one so we wont get an RFE.

I plan to read this whole thread so i will get familiar with the experiences of other VJ members and figure out what we should do. Its little bit scary when we read how much some of the CS`s charge for the i-693, so honestly we are worried.

Hi M18! You mentioned that you didn't submit the I-693 Form. Instead you only attached a copy of your DS-3025 and you got approved without any RFE. Now, I am thinking of doing the same. But I have a concern in my DS-3025 since it is incomplete and I need one more shot for my vaccination, i.e next year 2012. Although, they marked "Vaccine History Incomplete", but they also marked the "Applicant may be eligible for blanket waiver(s) not medically appropriate (as indicated above).

So I believe I can try my luck of just submitting a copy of DS-3025.

Just in case, could someone recommend a CS in South Florida which does not charge so high as I had inquired some CS's here, so far the charges are really high.

Hi I'm just new in this website.I will share my experience to you..I just got approved yesterday for our interview.and I didn't pass an I-693 form.I just pass the copy of my ds-3025..and they didn't request for that form.I think you will be fine.

Quick question

I looked up civil surgeons in Austin, TX on USCIS's site where they say you can find a USCIS approved civil surgeon and couldn't find Dr. L. E. Arnold.

Is this doctor USCIS approved? Otherwise, I don't expect USCIS will accept vaccination supplements issued by him, when applying for AOS.

If anybody knows anything about this, please let me know.

I saw posted elsewhere that Dr Arnold may have retired. This I693 is driving me crazy. Does anyone know if there would be a problem with getting the vaccinations and having the civil surgeon sign the I693 after the I94 has expired a few weeks? We are not sure what was turned in for medical. We have copy of the exam and xrays for visa and all was ok on that, but no mention can be found of vaccinations which have been had but early in childhood with no record. We are having a difficult time finding a CS in our area. The only one here is out on medical leave. We got married before 3 months but this i693 is causing us much stress. Thanks in advance.

Hi all! I am also in Texas, (El Paso) and need to file the vaccination part of the I-693. There are so many civil surgeons in the area and not one that will do just the vaccinations. I entered with a K1 visa and got married well within 90 days, and my medical was administered just a couple of months ago. I've gone through the thread with the CS list but I can't find anyone near our area. We have been through 10 doctors and clinics and no one will help us because they all want to perform the entire physical. We are pretty tired of this. I hope anyone in the area or at least within a couple of hours drive could help us. Thanks in advance.

So I went to one CS back in September and he also wanted to do the complete medical again and, even though my vaccination sheet was marked complete, he said I needed two more vaccines: against syphilis and tuberculosis. I was outraged cause those vaccinations weren't even on the fricken sheet to begin with :ranting: those CS are such bloodsuckers. Even when I presented them with the I-693 Instructions, they insisted. So I was like, f it, and filed for AOS with a copy of my complete DS-3025, and I also had a sealed envelope with my medical results that I had received separately in the brown envelope from the U.S. Consulate in Germany, which said to not open it and include it when filing for AOS, so that's what I did.

Got approved yesterday w/o RFE.

I was wondering, did anyone else also get a separate envelope containing their medical with a statement to send it in with AOS??

Congratulations on your speedy AOS!!

Your NOA and biometrics dates were within a couple days of ours, and we're still waiting for our I-485 approval (no updates and no RFE) although we received EAD/AP combo card earlier this month.

I appreciate your sharing about your AOS experience with the DS-3025. We did the same thing you did for AOS. We sent the DS-3025 ("Vaccine History Incomplete. Applicant may be eligible for blanket waivers...") my wife got from China. Here's a list of what she got for her k-1 medical in China:

  1. A white sealed envelope marked "USCIS Vaccination". According to the medical administrator in China, the envelope holds a copy of her official DS-3025 for AOS. (She did her medical prior to her interview, got a bigger sealed white envelope which she turned in during her k-1 interview. After her k-1 interview, she went for vaccination and got the smaller sealed envelope for AOS.)
  2. A copy of my wife's DS-3025 for her personal records.
  3. A yellow booklet "International Certificate of Vaccination or Prophylaxis" with my wife's passport photo and vaccination records identifying the shots she received.
  4. A white sheet titled "Appendix C Vaccines To Complete for Adults" listing doses she received and when the next doses should be given (booster shots)

For AOS, we sent the sealed envelope (DS-3025) along with photocopies of items 2 and 3 above. She did not get any booster shots nor see a CS for the I-693.

I hope our experience will benefit those who are adjusting status from k-1 visa in the future.

I am so confused about this process. My wife is from the Philippines and she did her whole medical exam there. Do we still need to find a CS or was that all handled in the packet she handed on POE?? Someone please help!!!

It's time for us to do adjustment of status also. I just started looking at this vacination #######. I'm feeling the same confusion. St lukes gave Judy a "Vaccination Documentation Worksheet"

that is marked Personal copy. I guess that is just a copy of one that went into the file that she turned in when she entered the country. My confusion started when I read the following from a thread called Life after POE.

. FIND A CIVIL SURGEON

Only if your DS-3025 is marked "incomplete" is a I-693 required to be completed. If it is marked as such (incomplete) you are required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

I have not found proof of this yet, but I'm thinking if your DS-3025 says "Vaccine History incomplete" and also "Applicant may be eligible for Blanket Waiver" then you don't need to do the

Partly correct. The required shots for AOS also have to be on the DS-3025. It has to be filled out correctly by the doctor (ie, marking which shots were not appropriate because of age or insuffucient time) and it has to be signed and dated.

I called Dr. Arnold's Office, but his number is no longer in service does anyone know the new number??

The Dr. Arnold talk is years old. He closed his office a couple of years ago.
