I've been a lurker and I must say LOADS of wonderful information, thank you to all who have advised and given input, very wonderful! Anyways, I signed up because I wanted to share my story (in an other place) but I had a question.

My wife (married 2/14/11) recently had her physical, and we have an appointment with a civil surgeon next month. At her physical she had her TB test as well as other required tests (per the I-693/Ds-3025). Basing from the information I've read here, there is no need for her to have a full medical exam from the CS, and all we need is her vaccinations/tests/med. information transcribed to the I-693, is this correct?

That would be wonderful because it would save quite a bit of money when the fees for all this rack up pretty quick as is.

If my vaccination document worksheey is ticked vaccine history incomplete and then ticked Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriated (as indicated above) but all the boxes are ticked showing the completed series that are required then all the other boxes are ticked Not age appropriate.

so does this mean my DS-3025 is ok?

Not sure if this will help you or not, however, I had my medical done in Canada in Sept of 2010; I poe'd in Dec of2010. We filed for my AOS in February and I did not have the I-693 transcribed; I did however include a copy of my vaccinations received and the form I rec'd from Medysis who did my medical in Montreal. My case has been transfered to CSC and so far no RFE it. I also included a copy from their web-site listing the requirements concern9ing the medical as someone else posted on here for you to read. We just figured why spend the $ if it wasn't absolutely necessary. Good Luck!

I have a similar case as yours. I had me medical done at Medysis in October 2010, POE'd November 2010, got married that month as well and filed for AOS in December. I did not fill out the I-693 but sent the vaccination doc. worksheet the office gave me. No REF's and my interview is in a few weeks. I did get sent a checklist to make sure i bring all the documents required and the I 693 is on there. It says I 693 and vaccination supplement in a sealed envelope (unless already submitted)

So im not entirely sure if i should go ahead and get another medical done or not. The interview in Montreal I gave them all they asked for, so Im thinking that counts as "already submitted" or I would have gotten an REF....anyone have an opinion??

I am wondering the same thing. It is the varicella vaccine that I did not get. They told me that I didn't need it for the visa, but that I may need it once I applied for adjustment of status.

On the bottom of the vaccination chart on I-693, it says "Results:" and there are 4 check boxes. Each box corresponds to the following:

1. Applicant may be eligible for blanket waiver(s) as indicated above.

2. Applicant will request an individual waiver based on religious or moral convictions.

3. Vaccine history complete for each vaccine, all requirements met.

4. Applicant does not meet immunization requirements.

Before the CS sealed the I-693 in an envelope, I presume the CS gave you a copy of the vaccination chart. Which box was yours checked??

Hi I'm just new in this website.I will share my experience to you..I just got approved yesterday for our interview.and I didn't pass an I-693 form.I just pass the copy of my ds-3025..and they didn't request for that form.I think you will be fine.

Thank you for sharing your AOS approval without the I-693. I was thinking of doing the same for my fiancee when we're ready to file her AOS in a few months time. :)

I am wondering the same thing. It is the varicella vaccine that I did not get. They told me that I didn't need it for the visa, but that I may need it once I applied for adjustment of status.

Just an update, I have officially been approved! So, for anyone that is wondering, it is ok if you have not had the varicella vaccine.
