FAQs: Forms I-693 and DS-3025

Please can fellow VJ's help me understand this. We have applied for AOS for myself and 2 children , K1 and 2 K2's, we have received our NOA and Biometrics letter , biometrics we go for on 25 June 2010. What I dont understand is that today we get RFE's on the 2 children's I-485's. What the RFE letter says is :

Submit a properly completed and signed Form I - 693 , Report of Medical Examination and Vaccination Record for the applicant on Form I-485. The medical examination must be conducted by a civil surgeon who has been designated by the USCIS . The medical must be issued less than one year prior to the filing date form of I-485.

Now if you read Form I-485 instructions , page 4 , 7 medical examination it says if you have had medicals done in last year you do not have to have them done again , we had our medicals done last October before coming to USA, so surely we dont have to have them done again ? Also the surgeon here in USA did our I-693 in May 2010 completed the whole vaccination records. When we phone USCIS all they say is they cannot see if they received all this information , which they must of to issue a NOA and a Biometrics , also say they cannot see what was wrong with the I-693 if anything was , surgeon says it was completed correctly, all they say is that it could of being a mistake but will make an enquiry and we should have a answer in 30 days, that makes us nervous , cos the RFE gives you 87 days to submit the RFE information requested . What do we do or what is the problem here ?? Pls Help !!!!!!!! Thank you.

We recieved an RFE for our I-693 even though we did the medical for the K1 less than one year since the time of the application. I called our local Representative's office and they contacted the CSC where our AOS had been transfered for processing and they basically told her his medical form was lost (not in his file were thier words) and so he must repeat them. I also sent in an RFE letter explaining this- but I have not heard back yet.

The bottom line is that we have to repeat the medical and I urge anyone who receives this RFE to do the same as unless you have a very helpful political contact or a lot of time on your hands to get to the bottom of where these medicals go (I am envisioning a file cabinet in a dark closet) than you will be better served by paying up and re-doing the medical!

Another $300 in doctors fees and lab tests! Yay!

Is this civil surgeon still in business? I tried the phone number and got a change of phone number message, which also did not connect? Any other good deal out there that will transcribe immunization?

Hello everyone

Well i had my medical exam in Egypt in March of 2009 and then i had to redo it again in May of 2010 cause i was under AP all that time .So my medical exam is still valid and i have a copy of the DS-3025 '' the vaccination '' .

My question is do i still have to file that form I-693 and do i still have to go to a Civil Surgeons to do that for me ? Or i will just have to send a copy of DS-3025 '' the vaccination'' when i will file for I-485 to adjust status ?

Some people say that i have to and some say i dont have to so im really so confused !!

I was reading that thread and it says i dont have to file that form I-693 and i will just have to send a copy of my vaccintation with the I-485 .

Hi, I just tried Dr Arnolds Office also and you are right, it is closed so in support of the previous posters enquiry - can anyone give us some reasonable civil surgeon names in Texas, preferably Austin or Houston who will transcribe our vaccination records without doing a medical. thanks

We called the CS in Tyler, TX (its the closest one to us) and they had no idea what we were talking about. Well, she did. The nurse said, well he needs to pay $145 for the full medical. I was like nooo, he has already had that, he just needs this form transcribed. And she still had no clue.

Anyone have any other way I could word it for them so they will understand? lol

I'm thinking of just sending in our AOS packet and hope they have his medical stuff.

Update: So I called the CS again in Tyler, Tx. Reworded my question. They are charging $50 to transcribe vaccination records. Thats a rip off imo.

Can anyone refers me to a certain Surgeon here in Pinellas or Tampa Fl??? I already injected 9 vaccines but i dont have ds 3025...

I have called several civil surgeons in my area re: the I-693A and none of them seem to know what they're talking about. The form isn't available online at the CDC anymore, so I'm stuck either just sending in the DS-3025 and hoping there was an I-693A in the brown envelope, or paying $140 to get things transcribed onto the WRONG FORM. UGH

Dr. Arnold IS retired, so no hope there. Anyone else got any suggestions?

Found one! Dr. Reeves at Concentra Medical will transcribe only for $25. His assistant Kelly is very sweet and helpful. I called EVERY OTHER CS in Oklahoma City (and a couple of other towns) and none of them knew what they were talking about.

We visited a surgeon in our area..he is charging us $180 then I need additional shot worth $50....I can't believe it is too much.....

Hi guys, maybe you can help me? I received an Rfe for my medical. Lettet said:

A review of all the applicants Uscis records fail to reveal form ds 2053, ds-3024, ds 3025 or ds 3026 which are Department of states medical form.

As such new form I-693 as such the applicant is required to submit a new form I-693 Report of Medical Examination and vaccineation record. That is properly completed and sign. The newest version of form

I-693 includes medical information as well as the vaccination record under th new guidance applicants two years or older is required to have a tuberculin skin test or blood test performed.

My question: Do I need to send them all the Ds forms or just the for I-693? all I have now is the ds-3025 this is my vaccinationss records from the phils which my civil surgeon transcribe for my first medical exam. this rfe is making me worried. Please help me. Thank you

I have the same concerns when it comes to AOS adjustment for Vaccination. I had my vaccination in Kuala Lumpur as K-1 visa, however when reading this post it looks like I need to do another shots here before I can send my adjustment application. In my DS-3025 it was clearly stated & tick the Td Vaccines and it was noted as " not routinely available", so it looks like I need to do shot for Td? I had my chicken pox and it was noted as VH in my DS-3025, so to make it safe I will do a shot for VH rather than bloodwork as i dont have proof ( by writting) or my vaccination book history with me only scars when i had the chicken pox..but I believe this will not suffice as a proof ( though i will still try my luck to show my scar just in case" ;-)..My question is can i do the shot to any doctors in my area? Anyone knows a doctors in apple valley minnesota can transcribed my vaccination to form I-1693?

I appreciate any info as I am in the process of consolating all the documents for submission ASAP.
