FAQs: Forms I-693 and DS-3025

I received an RFE for an I-693 despite having, what I assumed was, a completed DS-3025. I did call USCIS in a futile attempt to see exactly what the problem was, but to no avail; the two shots I need are marked and varicella is marked VH, all the other boxes are marked as not age appropriate.

I started ringing around Civil Surgeons in the Boston area and found that the first one I called was totally on the ball. They acknowledged that I didn't need a medical or any further shots and for me to "drop by and we'll transcribe the vaccinations onto an I-693 for you". All for the princely sum of $20.

Problem (whatever it is) solved (hopefully!)

Some of them refused to only transcribe the DS 3025, they also insisted on a full physical exam despite my explaination that I had a document from USCIS requiring only the vaccination part of the I-693. Needless to say, the $10 was a keeper!

It seems that the DS 3025 is no longer sufficient on it's own and that it needs to be transcribed onto the formal I-693 (part 2 vaccine chart).

$10! Nice find! I agree with you about the DS 3025 seemingly being no longer sufficient. What would be helpful when we received these RFEs asking for I-693s, is that USCIS could at least give us some indication as to why the DS 3025 submitted is not suitable. Like a lot of things in this process, I'm sure that will remain a mystery.

You need to submit a proberly completed Form I-693, Report of Medical Examination and Vaccination Record. The vaccination record must be completed by a civil surgeon who has been designated by USCIS."

So, this is the first time I am actually hearing about this form. Is this filled out by the docs from my home country, when I had the medical there? Was that in the brown envelope that I had to give to the immigration offficer when I entered the US? I did sent in my copy of the DS-3025 with my AOS application. I have read about transcribing this form at a surgeons office, which sounds not too bad. But I am wondering: How can they transcribe it, when the surgeon who does the transcription does not know about the results of the HIV test, the chest X ray, mental disorders etc. ??? Because the DS 3025 only talks about the vaccinations.

Also I am wondering if my husband should just get in touch with his congressman, to see if they just lost the papers.

Hope anyone can help me explain!

USCIS seems to be very inconsistent on this; sometimes accepting the DS-3025, sometimes requiring the I-693. The I-693 is for the complete medical. It contains five parts, part 1 is your info, part 2 is vaccination, part 5 is doctor's signature/confirmation. You need these parts completed (they are basically the vac records transcribed from DS-3025 onto the I-693). It has to be done by a civil surgeon.

I had already sent in my AOS package with just the DS-3025. We went to Family Health in Wautoma, WI today and had my I-693 vacc suppl. transcribed just in case we get an RFE. I had to have the H1N1 shot, he said it was recommended so I went ahead with it. Also, there was a discrepency with my mumps shot from years ago so I opted to have it again. It was cheaper then having the blood test to check for it. The whole appt cost $82 bucks. Not bad really, I thought it was going to be much worse. We got home an hour ago and my biometrics appt letter was in the mailbox. YIPPEE! Its not till January 8th in Milwaukee but at least its a date...website has no updates on my file at all.

Does anybody know any civil surgeon in the Los Angeles area that can transcribe the immunization record for reasonable price? I called Dr Arnold, shes not doing it anymore as 12-1 :( and I called one of the civil surgeons close by my house, she wants 110 dollar just to transcribe....

He works as a information specialist for a health care deparment, so he has heard a lot of bad stories already...scary stuff!

I think they meant they elected to have the h1n1 because "he said it was recommended", and I'm with you.. not recommended for immigration vaccines... just recommended for health reasons (in his opinion)

Now I understood! I think this whole USCIS stuff, I don't get it until I have read it at least several times. So they are only interested in the vaccine stuff and I don't have to worry about all the other stuff in Part 2. That sounds good, I will call a surgeon tomorrow and see who will transcribe it for me.

The doctor didn't say it was required, just recommended. We didn't want to take the chance to have it RFE'd for that so I just went ahead with it.

Hopefully, no one would get an RFE for a recommended (not required) shot, but if it made you feel better, then it was worth it.
