hi im from california and im having my interview for adjustment of status this june 25, i have not transcribe my vaccination yet..is it ok that Dr. arnold to transcribe my vaccination even if its in texas? thanks

Yes, any civil surgeon can transcribe them. At the interview, you may (but unlikely) be asked why you used an out of state CS. Just say that the one near all excessively overcharge and all want to do a full exam.

i have been reading this thread and i am still confuse...

can we process the AOS even if my vaccine is not yet completed? I had my vaccination even before i got here such as Tdap,HPV and MMR or should we finish the vaccination then we can process the AOS?

If you got at least one shot of a series of shots, you can apply for AOS. If there's 3 shots in a series, you don't have to finish three because there could be months of waiting to space the shots out. You just have to have the first one to send in AOS.

Question.

As listed in my signature, my husband entered the US on a K-1 back in April 2005. As part of the K-1 process, he had a medical exam and vaccinations with a consulate-approved doctor in Morocco in January 05. At the POE in April 05, he handed over his envelope. He didn't get copies of whatever was inside, so we have no DS-3025 (just a receipt for the exam).

After marrying, we applied for his AOS in August 05. This was less than a year after his medical exam in Morocco.

We have finally (honestly, just now, almost four years later) gotten an interview appointment for his AOS. It's a long story (and not our fault at all).

Anyhow. I've read the USCIS 'Application Procedures: Becoming a Permanent Resident While in the US' and noticed that the Form I-693 Medical Examination Sheet is "not required if you are applying based on continuous residence since before 1972, or if you have had a medical exam based on a fiancé visa".

However, as others have noted in this thread, the I-693 instruction pages suggest that, as a K-1 holder who had his exam overseas, we'll still need to submit Parts 1, 2 and 5.

Since his vaccinations were done in Morocco, the DS-3025 was part of his POE envelope, correct? Are we still required to see a civil surgeon about I-693 Parts 2 and 5? If so, without having a copy of whatever was in the brown envelope, how can a civil surgeon fill out that info?

I'm hoping he won't need another full exam and battery of shots before our AOS interview next month...

Advice, anyone?

I got my interview date on Aug 20 and on Monday I have an appointment with one of the civil surgeons serving my area (San Diego). They charge $100 for full medical with all the shots etc. And I told them that I already have shots done (completed using my insurance here). So basically, transcribing all the stuff for the I-693 is at a whopping $65. I just want to make sure that everything will be ok come the interview day coz I don't want my medical to be an issue for the delays of getting my GC.

I did submit I-693 with the AOS package but I left the CS certification blank but I provided a copy of my California Vaccination Record plus the DS-3025 (medical vaccination record copy done in the Philippines) and they didn't send me any RFEs for the I-693.

But again just to play safe, I'll head to the doctor on Monday afternoon so I can have her fill out the form. I'm not sure but I think this is a smart thing to do. :)

I have my AOS interview scheduled on the 24th of August. I did not include DS-3025 or I-693 in the package that we sent to Chicago and I did not receive any RFE's about it either.

I need 2 more shots,as stated on my DS-3025 and I will have it settled next week.I want to know,did you have a TB skin test too?And another thing,can you give me the name and contact info of the CS who transcribed your medical records?Im from San Diego too and planning to have the same CS transcribe my records as well.

We received the AOS interview letter. It has a checklist of what to bring listing the I-693 first stating ( if not already submitted). However, I sent the DS-3025 & 3026 with my AOS package and I never got an RFE for the I-693. Do I need to get on from a civil surgeon?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

I have a vaccination page from my medical that was a copy given to me and I have the xray on cd that I was told to keep at my interview. Is there anything else i will need for the AOS medical files? Should I ask for a whole copy at my POE or is it just the vaccination page that is needed. Slightly confused on this. Sorry its probably been asked before! I looked but didnt see it.

never mind i found where it says i dont need the full medical just the transcribed vaccinations, so the sheet should be fine tho i may have to get an extra shot.

Guys, does this doctor send a copy of your medical together with the sealed one or i should have asked for it additionally? Sent him all the paperwork today but forgot to mention the copy.

Hi Ewok, i want to ask about I-693, do i need to go to the local civil surgeon for medical and vaccine or any doctor can to it for paper work?it would be to expensive to do the medical all over again!!!

it this doctor on your post can just do it by writing and pay $10 would it be valid when the USCIS received my I-693?

Q: Do I need to complete another full medical to adjust status?

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status.

Q: I received an RFE for Form I-693 and my medical is less than one year old.

Sometimes the medical gets "lost" during the transfer. It is entirely possible that you may receive an RFE for the vaccination supplement (Form I-693), and possibly even for the full medical (Form I-693), even if the medical is current. Here is a helpful post from someone who was able to relocate a lost medical.

To find a Civil Surgeon in your area you can link here to use the USCIS Civil Surgeons Locator.

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

Something to note if you are yet to have your medical: If you have not yet had your medical prior to visa issuance, make sure to ask the Panel Physician if they will supply you with a copy of your full medical prior to scheduling. If they will not provide one for you, call another Panel Physician. If you cannot find a Panel Physician who will provide you with a copy of your medical, or if you have already had your medical and were not supplied with a copy, try asking the officer for one at the POE.
