My belief is that the TB skin test is not required for all countries (though I am not 100% sure). Can anyone clarify? This might be new-thread material...

There are no requirements that pertain only to certain countries, everybody has to fulfill the same requirements.

We're in the same boat...medical in early June. People aren't on the same page...USCIS, Center for Disease Control, Civil Surgeons, and Panel Physicians in the foreign countries. I think we're just caught in the middle of the changeover. Maybe it will get better for those coming after us. Two medicals is extremely annoying. We are just waiting for an RFE on the skin test, or the I-693, or that our medical is lost like so many others. Didn't want to pay for transcription, only to find that they "lost" our medical. We'll see what happens down the road.

i am so confused. if i entered the us with a k1 visa, i only need to do the vaccination section of the i693, which just means transcribing from my vaccination worksheet from my previous medical exam. right? so where does the skin test come in? i don't think i had that done during my medical exam in the philippines. could anyone help clear things up for me?

and by the way, i've already submitted my aos application because i read about these rules a little too late. so basically i'm doing this i693 in the case i get an rfe, which i hope i never get.

With the new rules, it seems that everyone applying will need to file a I-693 signed by a CS.If the medical tests were done overseas and within the last 12 months, then the applicant doesn't need to have the medical done again, however a tb skin test is required (most doctors overseas do the chest x-ray it seems) and the test for hiv/aids/syphilis. Just need to get a CS to transcribe the information on to the 693 if the applicant has the information from the doctor overseas (ds-3025)

Seems like they are starting to deal with the new I-693 form. An RFE received this week:

New Medical Form I-693: Please submit a properly completed and signed Form I-693, Report of Medical Examination and Vaccination Record. A new version of the medical form I-693 (Revised 06/05/08, or after) is required to be submitted by any applicant whose medical examination was conducted on or after June 01, 2008. The old version of the medical Form I-693 will not be accepted if the medical exam was conducted in or after that date. The medical examination must be conducted by a physician who is on the list of Civil Surgeons approved by USCIS.

Folks,

my wife and self(sponsorer) had an interview at the local USCIS office. During the interview, it was noticed that the TB skin test was not done, so the adjudicator asked us to get it done but remarked while taking away the I-94 that everything looks is set for approval pending the resubmission of the TB test.

I have a question about the medicals. My husband got his in march of last year. does it expire in march on the particular day he got it? would it be too late to use it if we send it in feb? do the count it as the day they receive it or the post mark?

I have read and re-read the instructions for form I-693 and if applied to the letter, it means that NO ONE on a K-1 visa needs a TB Test (probably because everyone gets an X-Ray and the doctor would state something regarding TB if that was positive).

The Civil Surgeon needs to fill out the vaccination section (transcript from the section prior to K-1) and sign the document, put it in a sealed envelop and give it to you.

I just called a local CS and she quoted me $60 for the transcript and $15 for the TB test (if we want it).

The important words in the new instructions are:

If you were admitted as a K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e),

you received a medical examination prior to admission, then:

You are not required to have another medical

examination as long as your Form I-485,

Application to Register Permanent Residence or

You will, however, be required to complete Part

1, Information about you, and submit the

vaccination section of Part 2 with your

We agree there is a lot of confusion, in part because the form I-693 is generic and used for many types of immigrant visas.

I just called the USCIS and after 15mn on the line, they were not able to answer my question and referred me to the Chicago Lock box address (no phone number available).

I also called a local Civil Surgeon and she was not overly aware of the requirement for the TB test, she said she would fill out the vaccination section for $60 (includes a required visit appointment).

There must be a few K1-K2 people who had their medical after June 1st who must have been through the AOS RFE/Interview phase by now and can give us their experience.

Not many I must admit since it takes about 6 months to get to the interview after filing, so filing would have had to be done no later than mid-August.

Please don't pay $110 for just transcribing your vaccinations onto a I-693. As recommended by other VJers, I sent my DS-3025 to a civil surgeon in Austin, Texas who will do it for $30 (includes postage too). Mine came back after about a week.

They are used to providing this service so you don't need to explain painfully that you don't require another medical exam, provided you've completed all the required vaccinations. They will call you back after you leave a voicemail.

I could use some guidance.

Yesterday I received an I-797E, RFE for my I-485. This is what my notice states:

MEDICAL EXAMINATION (FORM i-693)

NOTE: All medical examinations and vaccinations must be conducted and administered by a physician who is on the list of Civil Surgeons approved by USCIS. To obtain the names and telephone numbers of the designated civil surgeons for your area, please call the National Customer Service Center of the USCIS at 1-800-375-5283. You will be asked to provide your ZIP code. You may also access the list from the USCIS web site at www.USCIS.gov. Click on "Immigration Forms, Fees, and Fingerprints," scroll to Form I-693, click on the form, then click on "FAQ's" and go to the link for "Civil Surgeon locator." ALL medical examinations MUST be submitted in an envelope that has been sealed by the Civil Surgeon.

The Medical Examination that was conducted overseas (in Canada) as part of your K-1 Visa packet states that you have a Class B condition that requires a follow up examination within one month of your arrival in the United States. Evidence of the follow up examination and clearance by the examining civil surgeon from any restrictions on the applicant's ability to support herself as a result of the Class B condition must be submitted. Therefore, please review the information below and submit the required evidence."

To start with, there is no other information to review. What I do know is that at the time of my medical in Canada I was on Effexor for depression. By April of '08, I was no longer taking the meds. My file is currently at CSC.

How do I reach these folks to find out what this "Class B condition" is??? Any suggestions????

Does Dr. Arnold accept a copy of the previous DS-3025 form (filled by the Civil Surgeon in the country of origin for the K-1 application) and copies of any other immunization record - like the yellow immunization card Angela got today for the Td-ap vaccine at the local county clinic - , or does she need the ORIGINALS?

And I was reading about them requesting "receipts". What kind of receipts do they want?

By the way, I will be writing to our dear government agency to mention the racketeering by so-called Civil Surgeons here in good old Bay Area. I went to three Civil Surgeons to get the vaccination Part 2 filled out and you wouldn't believe how much they were asking for... well, at least the last one said they were "no longer doing it" (a lie!). Of course, if you accept the physical examination at $190 and the Td-ap immunization at $65 ($20 at the local County clinic), they will transcribe for a low $25! Basically around here the Civil Surgeons do NOT want to do their jobs as instructed by the USCIS, so they should not be approved in the first place. Proves that the only reason these doctors are CS is for the money. These immigrants are easy targets, and there is no dealing with insurance, all cash, what a deal. Free advertising from the government and $200+ in the pocket for 5mn work.

"Racket": extortion carried on for profit.
