Hi, Gang...

This is likely a stupid question, but I can't seem to find anything definitive, and have found conflicting sources on the net...

Where do we get the I-639A that's required for adjustment of status? VJ FAQ seems to say she'll get this overseas with her medical, but several of posts say just the contrary. (Do we REALLY have to schedule an appointment with a USCIS civil surgeon? What a pain! Closest one is 100 miles away!)

If she can get it overseas, I'm assuming the physician can fill it out and give it directly to her? I know she has vaccination records of some sort (I believe they were required for her B-2)..

Hi Jesse,

Vaccination records are not requiered for a B-2 visa. They are required for K1. Your fiancee will attend a medical appointment before the embassy interview, that includes a regular check up, some questions regarding her medical history, some lab tests and she will be asked for her vaccination records. She should ask the physician to give her a copy of her vaccination records, as everything else will go in a sealed envelope that will be surrendered at POE.

With that copy, and once in the US, when it's time for you to adjust status, you will look for a local physician that will transfer her vaccination records to a I-639A. some doctors can do this for you through fax if the office is too far away (for example, DR LE Arnold in Austin). You won't find the form in USCIS' website, but they will already have it at the office so don't worry. it cost us $10 to have the records transfered to the vaccination supplement for AOS.

Thanks a bunch for the reply! I didn't even consider that this could be taken care of completely with phone and fax. In fact, knowing a doctor that will do it is a big plus, so thanks also for the name. I'll probably call Dr. Arnold before bothering with anyone else, and give them my credit card number and be good to go :thumbs:
