After numerous delays, snafus and fubars by all parties involved including myself I am now stuck at this point.

I can't find any documentation that resembles I-693. My then fiancee now wife had her medical exam in Toronto. What do I need to do if I can't provide this information.

You get the I693 from a civil surgeon and you have to show him your vaccination records (or a vaccination worksheet.) You can find what it looks like but it doesn't do you any good as it has to be filled out bu a civil surgeon - and you do that before you file for AOS. What phase are you in? You need to fill out your timeline. Good Luck

Technically, form I-693A is only supposed to available via a civil surgeon.

This is where I stand. I'm about to go broke because my wife with a masters degree in English can't get a job because of this immigration BS. We have now been married a year this AOS process has not moved a freaking inch.

I have no idea what a civil surgeon is. Is that a Canadian govermental appointment? She has already completed all the required medical exams in Toronto in the fall of 2004 prior to completing the K1 process.

Sorry to seem so upset but we are sending in documents now for the third time after following the instructions that I got from this website the first time. Is it just me or are the rules/ fees constantly changing? We are both just so frustrated.

Is this I-693 document something that I should have in the 2 inch pile of paperwork that I have from my K-1 visa filings. If it is I can't find it and I have kept meticulous records.

No the I-693 is filled in by a civil surgeon here in the US... it is also know as a vaccination supplement.... your wife takes her vaccination worksheet from her medical in canada to a civil surgeon and he completes the I-693 with the information... he then seals it in an envelope and you then send it in with your AOS application or RFE....

Please post your timeline so we now where you are in the process....

No, it is a form the civil surgeon will fill out and give you.

Forgive me, I'm unfamiliar with the term "civil surgeon" and I worked for a major metropolitan health department for several years. Since my wife is from Canada do we need to contact the "civil surgeon" in Ontario to aquire this document? Any idea of the amount of time it will take to get this information.l

A Civil surgeon is a USCIS designated Doctor, the form you need I-693 is a form this Dr will fill out.

call around though, the fee's the Dr will charge are seperate from the USCIS form fee's and the Dr's fees vary widely from one to the next.

K1 Visa

June 15, 2005 - Submited I-129F

June 29, 2005 - Received NOA1

July 09, 2005 - Received NOA2

July 19, 2005 - Package 3

August 18, 2005 - Package 4 with the interview date (october 17, 2005 in Rio)

September 6, 2005 - Medical exam done

October 17, 2005 - Interview date APPROVED!!!

December 2, 2005 - Arrived in the U.S.

December 8, 2005 - MARRIED!!!

AOS

December 23, 2005 Sent completed forms for AOS

Jan 2006 - Checks cashed for AOS

Jan 06, 2006 - Received all 3 NOA's

Feb 16, 2006 - EAD and AP approved

Feb 24th, 2006 - Received AP in the mail

March 18th 2006 - Still no biometrics letter, made infopass

April 11th 2006 EAD Biometrics

April 18th 2006 Received EAD in the mail

May 10th 2006 AOS Biometrics after calling 800 number for 2 months, they claimed letter was returned undeliverable by the post office.

May 25th case transfered to California Service Center

June 5th case received by California Service Center

June 14th - Email that a Greencard was ordered (no interview)

June 17th - Welcome letter comes in the mail

June 19 2006 - Greencard arriives in the mail - WE ARE DONE!!!

Sept 2006 - Passed the road test

October 2006 - First job in U.S.

March 2008 - File I-751 - Biometrics taken

The I-693 is the medical form and the I-693A is the supplemental for vaccinations. If you need to get the supplemental completed go to http://uscis.gov/graphics/exec/cs/Index.asp? and file a civil surgeon in your area. I suggest you shop around, the prices can vary dramatically.
