I had an Infopass appointment yesterday at the Des Moines office to get emergency AP, and after a fair amount of waiting, it was approved. I was given 3 sheets of paper, all titled 'Authorization for Parole of an Alien into the United States'. The form number on the bottom left is 'I-512', and is revision 10-1-82. This is a bit concerning to me as from the reading I've done on AP, most people seem to have form I-512L. Also, a form that is 24 years old makes me nervous.

All 3 sheets are identical as far as I can tell, but one of them doesn't have a photo stuck to it.

Does this sound right to you guys? I just want to make sure that I'm going to be able to get back in to the US. Getting turned back because they gave me an outdated form would really be upsetting.

I'm going to give the National Customer Service Center a call but I know how reliable their information can be sometimes, so I thought I'd check here aswell.
