Hi, had to take a break from the rigors of paperwork,right my query is the vaccination record I got from the UK is a little poxy orange booklet with my name on the front with no official stamp wher there should be one,

it does list the Tetanus Diphtheria dated 15/5/98 also the MMR dated 1/9/05 and signed by a nurse, is this all thats required or is there some other way of presenting this info.

The thing you need for the AOS is called a vacinnation supplement. This is filled in by a panel physician in the US and you send it as part of the package.

The information you have should be OK and the doctor should accept it and give you any missing jabs

you have to search at website for uscis doctor list in your area, pick one and ask for vaccination supplement form I-693A i think, bring all your records whatever you have and they will fill up the form for you

mine was similar to yours, from mexico and i had no problem, so it depends pretty much on the doctor filling up the supplement

As discussed in the forum earlier this week, you may want to call a couple of different doctors for quotes. Some Doctors are only charging $50, while others are charging over $100.

Removal of Conditions (I-751)

August 16/08: VSC received I-751

August 22/08: check cashed (certainly did not waste any time)

September 11/08: Received NOA 1

September 15/08: Rec'd Biometrics Appointment Letter

October 02/08: Biometrics Completed

March 04/09: File transferred to CSC

June 01/09: Letter stating Petition Approved and Card Production ordered

July 12/09: Card Production ordered e-mail

July 13/09: Received card in the mail

The link to the civil surgeon finder on the USCIS website is HERE. And definately do a search to find the best priced place, some members have been stung for loads of $$$'s as other people have pointed out!

Note: Dates are in UK format because I'm (still) stubborn!!!
