Hello,

We are just starting the AOS process and had a few questions.....

On the I-485, G-325 forms...

1. Now that my wife is married to me do I still use her maiden name on the areas that ask for family name or do I put her new married name?

2. Is the vaccination supplement required without exception? Reason I am asking is two of her friends have received their Green Cards and never submitted vaccination form.

22 Aug 2006 She received Driver's License

05 Sep 2006 AOS Interview (Green Card) Scheduled

15 Dec 2006 Sent letter to USCIS requesting status of AOS

02 April 2007 Received Green Card in mail

Hey Warren,

1. If your wife is taking your last name, then put her married name when it asks for family name. On the G325A (make sure it has an A at the end), where it says "Other names used (include names from previous marriage)" put her maiden name.

2. Thats odd, i know in the assembling instructions it states you need the I-693(A) when filing. I wouldnt chance it, you may get a Request for Evidence letter (which slows down your process) and have to get it anyway.

We used my last name for all of Claudeth's AOS forms. As far as the supplemental vaccination goes, if you have a vaccination record from your doctor that covers all the basic vaccinations you shouldn't need the supplemental from the civil surgeon.

It's whatever name she wants to use going forward. If she took your name, use the married name -- it would be a big hassle to change the name on the card in the future.

I would not risk it not sending the supplement. I think that in almost all cases it's being reviewed at the interview.

09-02-2005 Applications for AOS, EAD, and AP received by MSC

10-21-2005 AOS fingerprint notice for 12-08-2005

11-07-2005 AP approved

12-05-2005 Infopass appt at San Jose office for interim EAD -- Refused, because it is already approved by MSC on 11-07-2005

12-07-2005 Attempt at interim EAD at San Francisco office -- no go. Back to San Jose, where CSO (chief station officer) tells they will contact MSC via email to request permission to issue interim EAD

12-08-2005 Biometrics for AOS and EAD. Having no EAD appt letter was no problem (used EAD NOA)

12-15-2005 EAD arrived in the mail

12-24-2005 Received interview letter; interview scheduled 03-01-2006

01-28-2006 Received replacement SSN card in married name (5 wks since application)

03-01-2006 AOS interview -- approved; received stamp in the passport

03-13-2006 Green card arrived in the mail

I believe in the instructions it states even if you have a vaccination record, you still need to go to a Civil Surgeon to have that information transferred to I-693A. (which is vaccincation supplemental form.) Therefore, if you want to play it safe, you may still want to get this done, and include it in your filing.
