Hey folks - we went to one of the civil surgeons in the area to have our vaccinations transcribed to the I 693A (since our physicals back in Canada were less then a year ago).

First of all...we had the hardest time trying to explain to the receptionist / nurse / physician that we DIDN'T need full physicals. Either they just wanted $$$ from us (yes, I'm a wee bit cynical) or they were just dumb.

Secondly...they seemed baffled by the fact that we wanted the I 693A filled out using the vaccination form we'd gotten from Dr Seiden in Toronto. They chraged us $75.00 each ($150 in total...cripes) just to transfer it.

In the end.....they gave us sealed envelopes (gee I hope they contain what we asked for).....as if it was our physical results.

Anyone else have this experience. Guess I'll just write on the outside of the envelope what it contains and then send it along with the AOS stuff.

They're suppose to put the one being sent to USCIS in a sealed envelope. we were also given a copy, not in sealed envelope, for our records.

Yeah, the CS I went to also put my I-693A in a sealed envelope. She gave me a copy for my own records though.

There are alot of Civil Surgeons that want to soak your for full medical when you only need the supplement done. Insist on just the supplement, take the I-485 directions and point at the paragraph that says K-1 visa holders only need the supplement. I did and the fee went down to $35.

Yes, mine was in a sealed envelope also. They told me if there was any problems, or I needed a copy of that form, to call them and they'd resend it for nothing.

A lot of people have gone through this Dr. Arnold that YuAndDan have noted, and it's only $10.00. It's funny more people aren't using him! I didn't know about it, or it was just starting up when I got him, otherwise I would have used him. It cost me $50.00 here in the US for that. Biggest rip-off in this process as far as I'm concerned!

You got what you are supposed to have, do not open that envelope it goes to USCIS unopened, too bad they did not make a photocopy of it and give you one for your records. :wacko:

This is a widely asked question because there does not appear to be an exact way the USCIS will adjudicate this

Yes. My docs were handed to me in a sealed envelope, with a copy for my records. The I693A is for a full medical (or so I thought). My medical had expired, so I paid $125 for a full medical, only they didn't do a full medical only confirmed that I had the full shots, did the TB test, and tested my blood. The CS just added his signature to make it legal.

I feel Good...........like the moon is shining just for me!

yup..i had the same and they charged me $110 ...I would have asked around but I couldn't afford the wait. I need that EAD :) But yes....it's normal to have it in the sealed envelope..that's what they're supposed to do
