Along with the I-134 bring supporting documents such as:
1)Last years tax return (form 1040) along with copies of W2's, previous 2 years Income Tax Return copies or IRS Transcripts (available free on the irs.gov website) - for a total of 3 years of Tax Returns.
[Note: If you print out a copy to send to your fiance, be sure to SIGN the copies of the tax returns to make them official - not required with IRS Transcripts.]
2) Current letter from your employer (on company letterhead) stating your job is permanent (not temporary), your current salary and your start date.
3) Letter from your bank telling what type of account you have, when you opened the account, the average balance of the account over the past year, the current balance and the amount deposited in the past year.
4) Last 3 months bank statements.
5) Any additional evidence to prove your continued relationship.
Examples: If you met again after filing, include pictures, airline itinerary, boarding pass, receipts, etc. If you have not met again since filing, be sure to include any mail/envelopes, chats, phone calls, emails, etc. |

For New Medical Center AUH & DUBAI (Deira)
New Updated cost of medicals (New Test added which cost 450 aed ).check hospital website.
The cost for the US Medical for adult applicants is AED 1024. This includes consultation examination, x-ray, urine and blood test.
For urgent reports, mentioned earlier, the charges are more than AED 1024. Urgent charges are non-refundable in the event the test results are abnormal and further testing is required.
For children the cost for examination and blood test for tuberculosis is AED 650.
Vaccinations, if required, are charged separately depending on the type of vaccination required.
4 vaccinations cost 600aed.
if only 2 vaccines (MMR & TD) will cost AED 230

Original Passports and three copies of the first 2 and the last 2 pages of your passport
6 passport size photographs (35 mm x 45 mm) with white background, Vaccination Record in English (if any) & Embassy Appointment Letter,
