�Two (2) full color, 5x5 cm, front facing photos on glossy paper with a white background. Photos are required of all applicants regardless of age. Photos must be recent (i.e, taken within the last six months).
�Long-form/unabridged Birth Certificate(s) showing names of both parents
�Police certificate/s for any country the applicant/s resided in for six or more months
�Evidence of financial support (I-134 and current tax returns from petitioner)
�Proof of Relationship
�DS 160 (Application for Immigrant Visa and Alien Registration)
�Death Certificate: Death certificate of any previous spouse if applicable.

�Fees: currently, the visa application fee is $265.00, which will be required during the interview.

Please DO NOT go to a Dischem/Pharmacy and load up on all the vaccines on the required list below, as most are not age appropriate and/or seasonal and therefore not necessary. Bring your vaccination card (if applicable) to the medical. They will fax this to the Centers for Disease Control and Prevention, to see which vaccines you need. (Only takes 10 minutes).

Emerald Guesthouse and Airport Inn
Just around the corner from OR Tambo and provides a shuttle service to and from the airport free of charge. They also do a free shuttle service to and from Rhodesfield station where you can catch the Gautrein to Sandton (2 stops over). Sandton station is within walking distance (�750m) from the US Embassy.
