***Added 9/26/14 by Jorge (When I called the US embassy in Lima to try to speak with someone in the immigrant unit during the above hours the operator said that they DO NOT longer respond over the phone regarding immigrant or non immigrant questions, that I should email the unit directly instead which I did. Eventhough the message on the phone while you wait it says you can, the operator said it is an old recording.)*** SAME 12/30/2015---recording is old, they do not do anything with these calls anymore, you must send an email.

*** Added 02/23/2016 by Cheyenne5044 The USCIS office has open-office hours from 9:00AM until 11:00AM ONLY on Tuesdays and Thursdays. No appointment is needed.
Best Contact Method: By e-mail once you have a case number. You can email them and they usually respond the same or next day.

What the Checklist includes (Jan 2014):

- Fill DS-160 online form on the CEAC website.

- ***Added 9/26/14 by Jorge. Fill and bring form DS-157 if the applicant is a MALE only and between the ages of 16 and 45 yrs. Document must be filled completely, signed and dated.

- Passport

- Appointment Letter

- DHL registration on http://peru.usvisa-info.com website. (Must create an account and choose a DHL office to pick up passport. ***Added 9/26/14 by Jorge (If you have more than one applicant, example fiance has a child, add the name of child under fiance and select location of DHL office to pick up passport. This has to be done for each applicant getting a visa even if it is a child)

- Beneficiary's birth certificate

- Judicial Penal Certificate (CERTIFICADO JUDICIAL DE ANTECEDENTES PENALES): Can be issued at "Banco de la Nacion" after paying the fee. ***Added 9/26/14 by Jorge (Obtain this certificate at Poder Judicial through the Registro Nacional de Condenas located on Av. Abancay, cdra. 5, first floor. Hours: Monday to friday from 8:00 a 4:45 pm. This certificate is returned same day)

- Judicial Certificate of National Records (CERTIFICADO DE ANTECEDENTES JUDICIALES): You can get this at INPE Dirección: Pasaje Adolfo La Jara 234, you must pay a fee at "Banco de la Nacion" first. ***Added 9/26/14 by Jorge (Pasaje Adolfo La Jara 234, corner with Av. Caceres, Urb San Antonio, Miraflores. Hours: Monday to Friday from 08:30 pm to 4:30 pm. This certificate is returned same day)

- Criminal Records (CERTIFICADO POLICIAL): You must pay the fee at :Banco de la Nacion" and then choose a police station that issues them. ***Added 9/26/14 by Jorge (The main center is at Av. Aramburú 550 – Surquillo, Hours: from Monday to Friday from 8:30 a 5:00pm. This document is returned 24 hours later)

- Military Records (ANTECEDENTES MILITARES): If applicable.

- Two US passport sized pictures (5cm x 5cm).

- Marriage/Divorce Certificates.

- Affidavit of Support: Form I-134. (You can add additional information to support it, such as: last tax returns, W2, statement from employer, statement from your bank...)

- Medical Exam.

- Proof or Relationship.

- Translation of all documents that are NOT in Spanish or English.

1/31/2013: I spoke with Doctor Melinchon office, an adult may need up to 3 immunizations, maybe less. The maximum additional cost would be S/.370(~$145 USD)

Make your medical appointment after receipt of interview appointment letter. Must be done in this order.
1/31/2013: The medical reports can be mailed in via DHL post-interview.

(the secretary at this office is the rudest person I've talked to in my life! I had to call again and complaint to the doctor about how rude her secretary was and of course, ended up going to Dr. Corigliano)

31/3/14 - My fiance visited Dra. Mélinchon last week. She just moved a few weeks ago. Also, note that x-ray and blood test must be done in Dr. Sandro Corigliano's clinic in San Isidro. She also required a tetanus vaccine (he had proof of one less than 6 months old), the flu shot (he's healthy, and it's not flu season), and varicela (he had chicken pox when he was little). All were excessive. She was very focused on drug use and required a toxological test, even though my fiance (truthfully) denied using any drugs.

Update(Jan-2013): MY fiancee called and spoke with the Doctor directly several times.

Cheap and good hotels near the Consulate? Hostal Monterrico- not spotless but reasonably clean (all clean linens and towels), hot water and air or fans in room (no heat in winter- but not bad)
Very friendly and VERY safe. An older couple run out of their home. About $25 night double for private shower. Also offer cheaper rooms with shared shower and mini suites for slightly more. Reasonably close distance wise but allow about 30 min drive time w traffic. Book through Pacarma.com.
Note: owner speaks limited English but was not a problem as my fiancee of course speaks Spanish

-Photos available at embassy for S/10.00.
-Arrive early so you don't miss your appointment, but they take you in the order you arrive. Plan on average 3-5 hours there. They will keep your cell front in the front security area if you bring.
-If appointment letter does not arrive, you can go to the embassy and pick up a copy of your packet (Mon to Fri 9-10 am). ***Added 9/26/14 by Jorge (You can also email them to request an electronic version of it along with the instructions letter.)

DCF filing is allowed for US Citizen residing in Peru for at least 6 months. In my case they did not require legal residence (Carnet Extranjeria) but only proof of having been living in the country for 6 months. If you are not a legal resident but have been staying on a tourist visa, you can prove residency in Peru by presenting documents such as house lease, phone bill, job letter, etc. Also they require a Migratory Movement issued by DIGEMIN which takes about 2 hours to get and S/50 Soles (about US$18). The lady at the embassy told me they were not the police and were not interested in why I did not have legal residence. They have been very accommodating and extremely efficient.
- I was also permitted to file DCF without Carnet Extranjeria.

An error was made in our information when it reached the NVC. This was easily corrected. We contacted them by email and had a response within a few hours that they'd fix that at the interview.
I had read and heard conflicting requirements for payment of interview fees but they were paid in cash on the day of the interview.
