ALL DOCUMENTS IN FOREIGN LANGUAGES MUST BE TRANSLATED TO ENGLISH! Translates must be qualified.

You are advised not to make any travel plans or give up your job.

Packet 4 Contents (if known): Letter with interview date which you need to keep and take to the interview with you.
Forms to Consulate? ** MAKE COPIES OF EVERYTHING FIRST**

When you receive packet three, you should return the following ASAP:

- The enclosed application for New Zealand Police Certificate.
- Fill out online DS-160 (this online form now replaces forms: DS 230,DS156,DS156K and DS 157. Once you have completed your DS 160 online, print a copy of the confirmation page containing the AA# barcode and send to the consulate. Your NOA2 hard copy received by your petitioner will instruct you of the website to complete the DS 160 or it can be found travel.state.gov website
- 2 x additional passport photos (each 50 x 50 mm square).
- A copy of valid passports biographic pages. (page with your photo etc).

THEN ONCE YOU HAVE ALL REQUIRED DOCUMENTS SEND IN:

- Your Birth Certificate - the original and a photocopy. Birth certificate must show both parent names. (Original will be given back to you at the interview).
- Birth Certificates for children under 21, even IF NOT travelling with you. An original and photocopy is required.
- ORIGINAL certified copy of divorce or death certificate of any previous spouse(s) for both applicant and petitioner.
- Police Certificate for all places lived for over ONE year (12 months) since the age of 16.
- Evidence of Financial Support - I-134 - co-sponsors also accepted - but ALWAYS complete a form for the petitioner even if you have a co-sponsor.
- Pre paid courier envelop - A4, I recommend courier post A4 tracked package. SELF ADDRESSED.
- Proof of your own funds, bank statements etc + Current Resume.
- SIGN THE ATTACHED FORM TO AGREE YOU HAVE ALL THIS AND SEND IT IN.

NOTE: The consulate WILL NOT schedule your interview UNTIL you have sent ALL of the above documents to them.

Other Forms (if any)? You will take the following to you interview:

- Current Passport, ensure it has one year validty + all previous expired passports with USA entry stamps / visas in them.
- Evidence of On-going relationship (BRING THIS TO INTERVIEW DO NOT MAIL IN).
- FEE: $397.50- MUST BE PAID IN NZ CASH OR BANK CASHIER'S CHECK, NO EFTPOS OR CREDIT CARDS ACCEPTED! They do give change for cash.
- Evidence of relationship (photos, letters, etc) (dont ask for directly but freely accepted) updated letters of intent (not necessary but got positive response for providing renewed ones)

Approx $360 for medical, $71.30 for X-Ray and there are a number of vacciantions needed depending on age. The price ranges from $40-$80.
blood tests which cost $80-$220. Blood test costs will depend on whether a blood tests are required to determine your vaccinations history if this is not available. $15 courier fee for results to be sent from Medical centre to consulate

Vaccination requirements? VACCINATION REQUIREMENTS: Immigrant visa applicants are required to obtain certain vaccinations
(some of which are listed below), prior to the issuance of an immigrant visa. Panel Physicians are required
to verify that immigrant visa applicants have met the vaccination requirements. The required vaccinations
listed are dependent on the age of the applicant. You should take any prior vaccination record
with you to the consular physician, who will advise you which vaccinations/immunology are required
and whether you require further vaccinations:
MUMPS, MEASLES, RUBELLA, POLIO, TETANUS & DIPHTHERIA TOXOIDS,
PERTUSSIS (WHOOPING COUGH), INFLUENZA TYPE B (HIB), HEPATITIS B,
VARICELLA (CHICKEN POX), PNEUMOCOCCAL, AND INFLUENZA.
Cheap and good hotels near the Consulate? Oaks Hotel in Gore Street - Around $100 - $140 per night, $25 for a carpark. This is one block from Consulate.
