Calling this number, you get into a menu which will refer to the information number underneath for all visa-related matters. I never got to talk to an actual person, then again I used to call in the afternoon

You should call this number between 9-12 am. Choose option 2 for Dutch menu, then option 5 for the immigrant visa department. I got an answermachine at my first try, when i called again 5 min later, I got to talk to a real nice lady.

You could also call direct: 020-5755331 or 020-5755333 (the last number is the one to which you are being forwarded when you call the above number).

I didn't find this one very useful (either) to be honest, for it seems to be predominantly aimed at tourist visas; a near endless barrage of menu choices and info that didn't help me any, but did cost me an absolute fortune

Issues Packet 3?
Packet 3 Contains: Letter with a link (http://amsterdam.usconsulate.gov/iv_k1_pkt3.html) to the consulate website with all the forms. Also on that website you will find:
-Checklist, to be sent back when all requirements are met
-Vaccination requirements
-Photo instructions
Issues Packet 4? Yes

YOu must obtain a police-certificate (VoG) in your city, you can do it online or go to the city hall.

If not what's the waiting period? 3-5 workdays. You will receive an email from the consulate with the tracking information once it is mailed. Once mailed, it is typically one day for arrival.
Is the Medical exam same day as interview? Prior to Interview
Cost of Medical Exam? 145 euros and 50 euros for the chest x-ray. Vaccinations you might need range from 35 euros (DTAP/DKTP) to 150 euros a piece, on top of the cost for the exam itself. All fees are to be paid on the spot, in cash and Euro only

Word of advice: visit your family physician and see if you can get the vaccinations that you need there. They'll be covered by your health insurance, if not completely then at least for the most part.

If you haven't had a update to your DKTP/DTAP shot in the last 10 years, get one prior. The one at the medical exam will cost you 35 euro.

Additional Info? Be certain to read the instructions on security. Absolutely no electronics are allowed, including cell phones. If you need, check with the surrounding museums for lockers. You can rent a small locker for a 1 euro coin Stedelijk Museum, or a large one for 2 euro without having to pay to enter the museum.
