For each family, bring
* A passport
* Three recent photographs
* All medical records of vaccinations, prior illness or treatments. You must discuss your medical history and current conditions - pregnancy, diabetes, hypertension and other conditions and medications taken - with the physician.
The physicians are Indian citizens practicing locally, and the examination they give to visa applicants conforms to standards and terminology prescribed by the United States Public Health Service. Any attempt by the applicant to influence the physician's medical judgment will reflex unfavorably on the visa application.
