- valid passport
- birth certificate
- divorce certificate
- police report from all countries you lived for more than 12 months
- 2 photographs 2x2 inches
- medical exam report (they send instructions)
- DS-160 https://ceac.state.gov/genniv/
- certification of intent to marry within 90 days (for the Beneficiary) -only K-1 Visa
- Affidavit of Support I-134 + instructions (+IRS Transcripts)
- The fee $265,00 or equivalent in Czech Crowns (both in cash only)
When you have everything collected, call the embassy to schedule an appointment for your interview. (you can schedule the interview first, before doing the medical exam - not officially, but everyone is doing it) |
