- police report from all countries you lived for more than 12 months
- 2 photographs 2x2 inches
- medical exam report (they send instructions)

- intent to marry within 90 days -only K-1 Visa
- Affidavit of Support I-134 (+IRS Transcripts)
- The fee $325,00 or equivalent in Czech Crowns - I recommend to pay in advance through ustraveldocs and bring the confirmation page
All documents have to be translated into English.
The interviews are being scheduled through UStraveldocs.com so create a profile and fill it out to be able to schedule. You're only allowed to schedule once the embassy actually received the petition. As of 2022 they e-mail the petitioner stating they received the visa with the Interview and Medical Information attached. |

My Clinic, Gen. PÃky 430/26 is currently the only place to get your medical done at - medical fee 15,000 CZK (as of 2022) for adult. Medical Fee incl x-rays. |

- bring your passport, four passport size photos (czech size), vaccination card and complete medical history if possible, covid vaccination certificate + the dates of each shot, printed DS-160 and interview letter.
Call them asap, because the appointments are few and far between (at least at the end of 2022 that's how it was).
Doctor will be pleased if you have all your vaccinations up-to-date (don't forget about flu season, covid (boosters aren't needed) and tetanus+diphteria not older than 10 yrs).
X-ray is in another building down the street, they will give you a map.
No need to bring entire medical history if you're healthy, but they do definitelly want to see the vaccination records and kept a copy of it. |

A few lessons learned from my experience (11/2022):
If you cannot finish all the steps on ustraveldocs right away, wait a few hours/ a day. I was getting a strange error message and so tried another browser, which led to my account being frozen for 3 days! Turns out, my case just wasn't uploaded to their website yet (not what the error said though).
Call MyClinic as soon as you get the appointment email from nvc, and schedule an appointment. The wait could be 4+ weeks. That was a really unpleasant surprise to me and meant I had to reschedule my interview to a month later :/
You can change the interview appointment date once you log in and fill out everything and confirm the original appt date.
MyClinic says to schedule the interview at least 2-3 weeks after the medical, however I was told by the staff that it is usually done way sooner. Don't count on that however and count the 10 work days.
Appointments for CR/IR1 are on Tuesdays and Thursdays at 13:00 and 13:30
