Once we receive an approved petition from NVC or cable notification from USCIS we send to the beneficiary an “Appointment and Instruction Package,” which contains the date, time, and location of the beneficiary’s scheduled visa interview and a list of documents he or she must collect prior to the visa interview. The required documents are listed below, under the heading “Fourth Step — The Visa Interview.” The package also contains instruction about where to obtain the required medical examination, how to make an appointment for the medical examination, and how much it will cost. Visa interviews are by appointment only. Applicants who appear without appointments are not interviewed.

What forms should be send back to the Embassy:

On the day of the interview, your fiancée will fill need to bring two completed copies of the Nonimmigrant Visa Application (DS-156) and the Nonimmigrant fiancée Visa Application (DS-156K). These forms may be completed in either Russian or English and are included in the Interview Appointment and Instruction Package. Each dependent child also needs a completed Nonimmigrant Visa Application, in duplicate. Your fiancée and each dependent child pay a $100.00 non-refundable application fee on the day of the interview. This fee must be paid in cash in U.S. dollars only. Original documents, not copies, must be brought to the interview. Originals of primary documents, such as birth, marriage, and death records, are returned to the applicant after the interview.

What other documents are needed for the interview:

Your fiancée will be asked to present:

- Valid passports for the beneficiary and any dependent children
- Birth certificates for the beneficiary and any dependent children
- Proof of termination of any prior marriages of both petitioner and beneficiary, as well as evidence of any name changes
- A police certificate from the current place of residence of the beneficiary, as well as from any prior place of residence for six months or more since age 16
- Police certificate(s) for any dependent children over 16
- Medical exam results for the beneficiary and any dependent children. The medical exam must be conducted by the Embassy’s panel physician (International SOS Clinic). More information on how to schedule an appointment with the International SOS Clinic is included in the Interview Appointment and Instruction Package.
- Proof of petitioner’s financial resources to ensure that his/her fiancée and dependent children will not become public charges in the U. S. The consular officer will ask to see copies of the petitioner’s tax returns (Form 1040) for the previous three years, the Form-134 Affidavit of Support, and proof of current employment (either current W-2’s or a letter from the petitioner’s employer stating salary).
- Supporting documents verifying the existence of a relationship between the petitioner and beneficiary (this could include copies of correspondence, plane tickets, photographs, etc.)
- Two color photographs with white background on glossy paper. The photographs must be 5 cm square with the applicant facing the camera directly (both ears should be exposed); the dimensions of the facial image must measure about 30 mm from chin to top of head.

The clinic is open from Monday through Friday between 9.00am to 6.00pm (note that appointments cannot be made after 5 pm).

Please bring the following to your medical examination appointment: one picture 3×4 cm, your valid passport, your immunization history, and the appropriate fees (payable in cash Tenge/ or Dollars or by credit card — note that American Express is not accepted).

Cost of Medical Exam:

The basic examination fee for each applicant 15 years of age and older is $100, and $75 for applicants under the age of 15 (however if a chest x-ray is required then the fee will be $100).
