(I posted this elsewhere but haven't receieved a response yet so I'm trying here, my apologies for posting twice...)

I'm going to be filling in my Subject Access form today to obtain my criminal record information, and I need to supply two forms of ID. In my county's constabulary you can send clear photocopies of the ID, and I was thinking of sending my passport page and a bank statement. However, I'm a bit nervous posting something with my bank details on. Would it be acceptable to black out the actual account number and sort code and just leave the address info, or would that defeat the object?

Or would a birth certificate and passport be OK instead? I don't have a driving license or NHS information (that I'm aware of). It doesn't specifically say that a birth certificate is OK, but it would seem to fit the criteria:

'To help establish your identity copies of two different official documents, which between them show clearly your name and date of birth, must accompany your application. For example, a driving license, NHS medical card, marriage certificate and passport. Bank statement will be accepted with a valid copy of your passport. Clear photocopies are acceptable.'

It doesn't say that my documents have to contain proof of address (which I'm supplying elsewhere on the form anyway). Can anyone who's done this help?

I just provided the Northumbria constabulary with my long form birth certificate & a copy of my passport. They accepted that no probs.

Cant you just take the completed form plus your documents to your local police station? I did that and they just checked off that they had seen the documentation and what I provided and gave me everything back. They didn't take copies of anything.

Just go to your local police station with ID, Drivers Licence or Passport will do, thats all I did and then they sent the form off for me after I handed over £10.00 postal order.

This isn't possible in all areas. For example, in Thames Valley, they require it to be done by post.

Indeed, for Thames Valley I sent my actual passport by recorded mail, and they sent it back by recorded mail within a week of receiving it. I have done that twice with no issues.
