Vaccination paperwork?

Okay.. here we go...

Im from Edmonton, AB.. and i was a graduate of the Therapist Assistant Program at MacEwan College. In order for us to be accepted into the program, we had to have ALL of our vaccinations up to date. The paper the vaccination nurse gave me was submitted to my school program.

My question is this: What kind of paper work is needed to show that im completed vaccinated (i know i am)? and can i just call my family doctor and book an appointment with him in regards to getting vaccinated for US visa? Will he have the required paper work disposal?

Anything official will do. In Ontario, there's a government form (mini-booklet) that shows the dates, etc. You should get your family doctor to do any required vaccinations as it will save you $$$ and is acceptable to the panel physician. The Centers for Disease Control has a list of required vaccines. Consult their website for what you need. The USCIS uses the recommendations of the CDC for their requirements. Try the CDC website @ http://www.cdc.gov/vaccines/, they even have an interactive web quiz. Hope this helps.

I thought she was asking only about vaccinations. The panel physicians do the medical examination.

Canadian doctors usually are not versed in the intracacies of US immunization policies. Go to the website and print out the list for your family doctor to peruse. I was lucky, my parents' doctor had a pretty good idea of what was needed but he still appreciated looking at the printout I gave him. It just makes things much faster. :thumbs:

Only for vaccinations, if you're intending to get your vaccinations done ahead of time you can use your family doctor. For the required medical examination, you'll need to use the USCIS-certified panel physician.

if you go for vaccinations to ur family doctor it's cool...but you will have another vaccination supplement filled out by the panel physician WHEN you're going to go for the medical examination! You will have to bring any records of vaccination from your doctor to your medical appointment. You will NOT, however be able to replace the form needed by the embassy with the one from your family doctor. If those people that will be performing the medical examination will decide you need any other vaccines they are going to give them to you anyways...

Sounds like you had some type of vaccination sheet given to your school that certified you had your vaccinations up to date. I don't know who did it. I would just ask the school for a copy of what you submitted when you went to school. Did you look to see what it was that you needed in way of vaccinations for the USA?

99% of Canadians WILL NOT need any vaccinations, because they are all required by law to have been done while we were school aged. The only one you may need boostered is the T&D (tetanus & diptheria), which is suggested every 10 years. If you've already been vaccinated, you do not need to get vaccinated again.

If you do not have record of your vaccinations, your family doctor can draw blood to run titre tests. This will show that you have immunity to the list of diseases, so you won't need vaccination. If your titres show up that you do not have immunity, your doctor may suggest you get the shots. In Canada its usually no charge if your regular family doctor does the shots, but if you get them done at the panel physician, they'll charge you for them.

I'm originally from Saskatchewan and I just called Saskatchewan Health and they are able to give me a copy of all the immunizations I have had. Not sure if it works the same with Alberta health but if your doctor was the one who gave you all you up-to-date immunizations, he should have the paperwork.

Capital Health no longer had my vaccination records. After so many years, they don't keep them anymore.

I just went over to the Medicenter and asked to sent to the Lab for a titres test to be checked for MMR and tetanus/diphtheria. I didn't ask to be checked for Varicella (chicken pox) The Civil Surgeon will accept oral testimony if you have had chicken pox. Plus I did have a letter from my childhood Doctor that confirmed I had it. The Civil surgeon here in the US when I got my vaccination supplement also accepted oral testimony that I had chicken pox.

I'm glad I went for the titres. I showed low immunity on a few things and ended up getting all my shots redone.
