AOS - Morocco specific question

Some of the terminology used in this thread is pretty poor. Sorryif anyone takes offense to this however, so much of the verbiage is incorrect, and being that this thread also now shows up under google searches, corrections need to be made (of course there is probably some that could be corrected of my reply as well, which is why the AOS forum is best for this kind of post)

Thanks for catching all this. I must have mis-typed and/or just assumed the OP typed the correct form number.

The DS 3025 is the vaccination supplement to the medical that the applicant should request a copy of from the panel physican during the medical exam. This form states clearly that a copy of the form should be given to the applicant. This form is then taken to the civil surgeon to complete the medical supplement form of the I-693 as required for K-1 applicants.

The supplement form to the full medical has been referred to as 1-693a on VJ many times. I just pulled the form and it does not have this label that I could find.

Yep - I have no idea where I-693a started from. I can only surmize it is due to being an "appendix A" of the I-693. I would guess most people call it that because others are, and we often see - "I can't find the form I-693a??? Where do I get that?". Its just a misnomer.

I think you are spot on. I just did a search. It's used over and over on many boards, but then I found a site that mentioned exactly this point- an invented term to refer to the supplement based on "appendix A".

I also think your point about this being a question for the AOS forum is an important one. MUCH better info there as this part of the process is not region specific. That said, I think the OP's question in MENA was good in that it allowed us to make that point! (Thanks Jen ;) )

The form might state this clearly, but they obviously don't follow the guidelines. Hamid was given no "forms" that were numbered, even after I called the doctor (that Casa requires). He was lucky to be given a copy of his shots, such as it was.

It might be worth mentioning that I have heard of no one going through Dr. Khadijah's clinic having a problem getting this form when requested. The problem seems only to occur with one or more of the other panel physicians.

The medical is good for one year. The AOS instructions state clearly the K-1 only needs the supplement vaccine sheet not the medical. You can try showing that to them. A lot of people use the doctor in austin I think?) , fax the info and send it back to you for a low fee.

Are the doctor's askign you to pay for an office visit? That might explain the cost.

Jen- We used a doctor in Santa Maria- about an hour North of SB and basically it took 10 minutes and we paid $10 dollars. Do you want their number? I'm sure you can send it or fax it to them. Let me know!

I have been working with a doctor's office for the past few months and now that I think about it I know that one of them is a civil surgeon. Let me ask him if he will do it and then you can just fax the stuff to me and I can fax it back to you or send it if you want.
