Timeline

in DS 157 question no. 9. List All Countries You have Entered in the Last Ten Years (Give the Year of Each Visit)

ive never been out of the country, should i answer NONE?

and in DS 156 question no. 7 First and Maiden Names (as in Passport)

let's say my name is Anna Cruz Martinez (Anna is obviously my first name, Cruz is my mother's maiden name and Martinez as my last name) should i put Anna Cruz in question no 7? and put NONE in question no. 8 which is Other Surnames used (Maiden, Religious, Aliases)

any input will be appreciated here, thanks!

NONE is always the correct way to fill the blank, not N/A.

If you have still been using Margarete fill in all the names with surname as last name. If you are using the normal form of first, mother's surname, surname, fill that in with Claudine Barretto in first and Santiago last, then use the birth name where it asks for other names.

got another question po...

I tried reloading the DS-156 online and it gave me another barcode, different from the first one i had already saved as pdf file. i want to edit my data but each time i do, i get a new barcode. will it be a problem if i had generated 2 or three different barcodes?

DS 156 is really confusing because we really need to make sure everything is correct. and it is also confusing cuz we hardly put doubts in ourselves fillin it up...hehehehe Speaking out from my own experienced. but i guess just know what is right and if still there's wrong in it as everyone says be confident and stand for it to answer with all your good but polite defense.

Anyways, for me i printed out DS 156 not in BACK to BAck format. cuz it says you'll be needing it in 3 pages including the Barcodes in the last page.

Question On DS 156 # 7: CLAUDINE MARGARETTE BARRETTO = First name and middle names (as in Passport)
