Maiden name on passport

My wife and stepdaughter went to a travel agency yesterday. They told her that she needs to use her family name as her middle name now when she applies for her passport,or she'll have trouble getting it. Unfortunately we've been using her Mom's ( or old ) name still as her middle name for the petitions etc. Do we keep everything "as is" with her name, and hope they issue her a passport with the old middle name, or do we now use her (new) maiden or middle name even though it will be different than what the embassy will be seeing with the paperwork coming from the NVC ?

Excellent and important question! I wish I could offer some info, but I am also interested to hear from those with actual experience in this. As a note, you may want to see a POLL that I posted regarding how many people changed their name when they got married and what variation of change they used. Interestingly, only 2 people out of 28 (so far) indicted that they made the change (which I had previously) thought was most common : Change existing middle name to existing last name and change existing last name to hustband's last name.

Claudeth had to get a new passport, the old one was expiring, and we got it in her married name with no problems other than all the Filipino red tape :blink:
