1. I am 30 years old and have lived at number of different places at different cities with a stay of more then 8 months. I had been at my current residence for a year now(Will that address be sufficient for the police clearance). Or do I still need to collect police clearance certificate from all these different places , which I think will take more then two months. And if yes do I have to go to all these places myself to collect these or the police stations would just send it themselves.

2. My fiance just needs to send me 1040 and W2 for the year 2004 as affidavit of support with other documents as per embassy checklist. Is that okay. Please refer to the checklist text given below.

3. I have filled the Form DS-230 Part 1 in adobe acrobat. Can I just send the print out of this form with my signatures.

The following text is from the checklist in Packet 3 that I received from US Consulate, New Delhi. INDIA

Police Report:"Each applicant aged 16 years and older must submit a police cerificate that includes information regarding the applicant including all arrests, the reason for the arrest(s) and the disposition of such cases. (A) Indian citizens residing in India for six months or more since turning 16 years of age must present Indian police clearance certificates from both their district police station and the local area passport office. Applicants whose passports were issued within six months of the visa interview may present just the passport office certificate. These certificates are valid for one year from the date of issuance."

Affidavit of Support:"Affidavit of Support (I-34), job and bank letter, 1040 and W2 for the year 2004 from the petitioner."

1 - The police certificate is required for any residence over 6 months since 16 years old, so yeah, you need that. I think it needs to be personal but I am not entirely sure, so wait for more advice on this.
