Don't sweat all this too much. A lot of folks don't follow the suggested format what so ever and get by just fine. Personally, I made mine following the suggested guidelines from the USCIS site. I used the Acco fasteners and in my case wrote "Resubmission" on my cover page since this is the second petition I sent for my fiancée (We had to withdrawal her first a few years ago.) I did not use tabs either time. I used a simple table of contents. Don't worry about mistakes in order and presentation. It's the content that counts!

Looks good to me. I followed the instructions here on VJ and the petition blew right through CSC. I also send Claudeth a complete copy of what I filed.
